中医按摩配合早期运动对ICU机械通气患者肌力及胃肠功能恢复的影响

Effect of Traditional Chinese Medicine Massage Combined with Early Exercise on Muscle Strength and Gastrointestinal Function Recovery in Patients with ICU Mechanical Ventilation

目的探讨对ICU机械通气患者实施中医按摩配合早期运动,分析其对患者肌力及胃肠功能的影响,旨在为临床该类患者的护理提高有力依据。方法选取医院2018年5月-2019年7月收治的76例ICU机械通气患者为此次研究对象,按照随机数表法分为对照组、观察组,每组各38例患者,对照组患者接受常规护理(创造舒适度环境、积极对症治疗、气道湿化、基础护理等专科护理),观察组患者给予常规护理基础上联合中医按摩配合早期运动(一指禅方法按压天枢、神阙、百会、合谷、中脘,针对患者病情给予相应运动指导)。对比分析干预后,患者肌力、胃肠功能及自理能力。结果相较于对照组,观察组肌力恢复情况较优,差异有统计学意义(Z=3. 120,P=0. 002);观察组胃肠功能情况优于对照组,差异有统计学意义(Z=3. 303,P=0. 001);观察组自理能力恢复情况优于对照组,差异有统计学意义(Z=3. 412,P=0. 001)。结论 ICU机械通气患者在常规护理基础上实施中医按摩配合早期运动,能够显著促进患者肌力恢复,有效改善胃肠功能,提高自理能力,值得在临床行机械通气的患者护理中提供有力依据。

Objective To investigate the effect of traditional Chinese medicine massage combined with early exercise on muscle strength and gastrointestinal function in patients with ICU mechanical ventilation. Methods Seventy-six patients with ICU mechanical ventilation treated in our hospital from May 2018 to July 2019 were randomly divided into control group (n =38) and observation group (n = 38). The observation group was treated with traditional Chinese medicine massage combined with early exercise. The muscle strength,gastrointestinal function and self-care ability of the patients were compared and analyzed. Results Compared with the control group,the muscle strength of the observation group was better than that of the control group and the difference was statistically significant. The gastrointestinal function of the observation group was better than that of the control group (Z = 3. 303,P = 0. 001) and the recovery of self-care ability of the observation group was better than that of the control group (Z = 3. 412,P = 0. 001). Conclusion The implementation of traditional Chinese medicine massage combined with early exercise in patients with ICU mechanical ventilation on the basis of routine nursing can significantly promote the recovery of muscle strength,effectively improve gastrointestinal function and improve self-care ability.
