摘要
目的检测特应性皮炎(AD)婴儿及正常婴儿头面部糠秕孢子菌感染情况,探讨糠秕孢子菌在AD婴儿发病中的作用。方法采用酶联荧光法分别镜检AD组250例婴儿及对照组60例婴儿头面部糠秕孢子菌。检测AD组婴儿外周血嗜酸性粒细胞计数及血清总IgE水平并计算客观特应性皮炎评分评估婴儿AD病情严重程度与糠秕孢子菌感染间的关系。结果 AD组婴儿糠秕孢子菌感染率显著高于对照组(P<0.000 1),两者阳性率分别为36.40%和8.33%,且两组糠秕孢子菌感染率均与性别无关(P>0.05)。1~6个月AD婴儿头面部糠秕孢子菌感染率显著高于7~12个月(P<0.01),对照组婴儿不同月龄间阳性率差异无统计学意义(P>0.05)。外周血EC增高组较EC正常组AD婴儿头面部糠秕孢子菌感染率显著增高(P<0.01)。血清tIgE水平增高组与tIgE水平正常组AD婴儿头面部糠秕孢子菌感染率差异无统计学意义(P>0.05)。AD婴儿头面部糠秕孢子菌感染与O-SCORAD评分之间无相关性(P>0.05)。结论 AD婴儿头面部糠秕孢子菌感染率明显增高,提示糠秕孢子菌在婴儿AD发病中可能起到一定作用。
Objective To detect the Malassezia Furfur(MF) infection on the head and face of infants with atopic dermatitis and normal infants,and to explore the role of MF in the pathogenesis of AD infants.Methods MF was examined by enzyme-linked fluorescence microscopy in 250 infants in AD group and 60 infants in control group.The peripheral blood eosinophil count and serum total IgE level were measured in AD group,and the Objective Scoring Atopic Dermatitis(O-SCORAD) index was calculated to evaluate the relationship between the severity of AD and MF infection.Results The MF infection rate of infants in the AD group was significantly higher than that in the control group(P<0.000 1),with positive rates of 36.40% and 8.33%,respectively,and the MF infection rates were independent of gender(P>0.05).The infection rate of MF on the head and face of AD infants at 1~6 months was significantly higher than that at 7 to 12 months(P<0.01),and there was no difference in the positive rate of infants at different months in the control group(P>0.05).The infection rate of M.furfur on head and face of AD infants in increased peripheral blood EC group was significantly higher than that in normal EC group(P < 0.01).The infection rate of MF on head and face of AD infants in the group with elevated tIgE level and the group with normal tIgE level showed no difference(P>0.05).There was no correlation between head and face MF infection and O-SCORAD in AD infants(P>0.05).Conclusion This study showed that the MF infection rate on head and face of AD infants was increased significantly,suggesting that MF may play a role in the pathogenesis of AD in infants.
