国医大师郭诚杰学术思想之“乳房当为奇恒之腑”论

目的总结国医大师郭诚杰教授的学术思想,为建立乳房在脏象学说中的地位提供理论和临床实践依据。方法通过研习郭大师对乳房及其疾病的认识。方药和针刺(含电针)治疗所发表的论文、出版的著作、临床病例的整理以及与郭大师生前的访谈等途径整理、总结、结果乳房与多条经脉相关,主要与脾胃肝肾关系密切;乳房形体中空,似腑;孕期“藏而不泄”“满而不实”,似脏;哺乳期生乳、藏乳、排乳呈现“藏而不满”和“传化物”的似脏又似腑的脏腑双重特性;非孕。非哺乳期育龄妇女之乳房呈现出不藏又不泄的非脏非腑特征;治疗乳房疾病当“以通为要”“补养以助”。结论乳房当为奇恒之腑。

Objective To provide theoretical and clinical evidence so as to adequately position the breasts in the Zang-fu theory based on a summary of Guo Chengjie’s academic thoughts.Method Doing a research on Guo’s recognition of the breasts and the diseases associated with them as well as the treatment including prescriptions and(electric)acupuncture by sorting out his published papers,masterpieces,clinical cases,and the interview with him.Result The breasts are associated with multiple meridians,mainly with the spleen,stomach,liver and kidney;They are hollow in form which conform to Fu-organs,while to Zang-organs when they have the properties of“storing but not discharging”and“plump but not excess”in pregnant women.They are characterized by“non-excessive storage”,and“transporting and transforming”in the phases of generating,storing and ejecting milk,which were similar in function to Zang and Fu simultaneously.By contrast,the breasts of women who are non-pregnant and non-lactational are characterized by“non-storable and non-dischargeable”,which are inconsistent with Zang and Fu.Therefore,treatment of breast diseases should put“promoting lactation”in the first place and tonification as complement.ConclusionThe breasts should be positioned among the extraordinary Fu-organs.
