脑出血与胆固醇酯转运蛋白表达水平及基因多态性相关性研究

Study on correlation between intracerebral hemorrhage, cholesterol ester transporter protein expression level and gene polymorphism

摘要 目的探讨患者发生脑出血(Intracerebral hemorrhage, ICH)与其胆固醇酯转运蛋白(cholesteryl ester transfer protein,CETP)表达水平及基因多态性的相关性。方法将本院2016年2月至2019年8月收治的150例ICH患者纳入研究样本,作为观察组,另选取同时期体检健康人员150例作为对照组。所有研究对象入组后予CETP相关指标的水平检测,并采用特殊方法测定CETP TaqIB的基因型。将两组患者的所有检测结果进行对比,以探析ICH与CETP TaqIB表达水平及其基因多态性与脑出血的相关性。结果观察组与对照组在性别、年龄、BMI、吸烟饮酒史、高血压史、心脏病史、TG、TC、LDL-C和LP(a)水平的比较上差异并不显著(P>0.05);而观察组的HDL-C、Apo-A1水平显著低于对照组(P<0.05),其Apo-B水平显著高于对照组(P<0.05);两组患者的CETP TaqIB基因频率及等位基因分布有显著差异,观察组的B1B1、B1比例均显著高于对照组(P<0.05);Logistic回归分析结果显示,HDL-C、Apo-A1、Apo-B水平和基因型均为导致ICH的独立危险因素。结论 CETP的表达水平及CETP TaqIB的基因多样性与ICH的发生存在相关性,其中HDL-C、Apo-A1、Apo-B以及CETP TaqIB基因频率均是影响ICH的独立危险因素。 Objective To explore the correlation between the occurrence of intracerebral hemorrhage(ICH) and the expression level of cholesterol ester transfer protein(CETP) and gene polymorphism in patients.Methods A total of 150 patients with ICH treated in our hospital from February 2016 to August 2019 were included in the study sample as the observation group, and another 150 healthy people who underwent physical examination at the same time were selected as the control group. All subjects were tested for CETP-related levels after enrollment, and CETP TaqIB genotypes were determined by special methods. All test results of two groups of patients were compared to analyze the correlation between ICH and CETP TaqIB expression levels and their gene polymorphisms and cerebral hemorrhage.Results There was no significant difference between the observation group and the control group in gender, age, BMI, smoking and drinking history, hypertension history, heart disease history, TG, TC, LDL-C and LP(a) levels(P>0.05). The HDL-C and Apo-A1 levels in the observation group were significantly lower than those in the control group(P<0.05), and their Apo-B levels were significantly higher than those in the control group(P<0.05). There were significant differences in CETP TaqIB gene frequency and allele distribution between the two groups of patients. The B1 B1 and B1 ratios in the observation group were significantly higher than those in the control group(P<0.05). Logistic regression analysis showed that HDL-C, Apo-A1, Apo-B levels and genotypes were independent risk factors leading to ICH.Conclusion The expression level of CETP and the genetic diversity of CETP TaqIB are related to the occurrence of ICH. HDL-C, Apo-A1, Apo-B and CETP TaqIB gene frequencies are independent risk factors affecting ICH.
