百令胶囊联合胰激肽原酶治疗早发2型糖尿病肾病Ⅲ期的临床观察

Effects of bailing capsule combined with pancreatic kininogenase on early-onset type 2 diabetic nephropathyⅢperiod

目的评价百令胶囊联合胰激肽原酶治疗早发2型糖尿病肾病Ⅲ期的临床疗效。方法收集2014年3月至2016年3月于安阳市人民医院内分泌科住院的早发2型糖尿病、肾病III期患者170例,对比胰激肽原酶组(单药组)85例与百令胶囊联合胰激肽原酶组(联合组)85例的临床资料,分析百令胶囊联合胰激肽原酶治疗早发2型糖尿病肾病Ⅲ期的效果。结果①治疗前两组间性别、年龄、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、体质量指数(BMI)、C反应蛋白(CRP)、血肌酐(Scr)差异无统计学意义(P>0.05)。②与治疗前相比,两组患者24 h尿蛋白水平较前明显下降(P<0.01),联合组下降优于单药组(P<0.01)。③治疗后两组患者TG、CRP水平下降,联合组下降优于单药组(P<0.05)。④治疗期间联合组仅有2例出现轻度腹泻,继续用药后缓解。结论百令胶囊联合胰激肽原酶降低早发2型糖尿病肾病Ⅲ期蛋白尿水平疗效显著,且能改善早发2型糖尿病患者血脂代谢,延缓糖尿病肾病进展,且不增加不良反应。

Objective To investigate the effect of combined therapy with Bailing Capsule and pancreatic kininogenase on the levels of urinary albumin excretion rate(UAER)for the eraly diabetic nepgropathy.Methods Totally 170 patients with the early-onset type2 diabetic nephropathyⅢperiod were collected from March 2014 to March 2016 in The People’s Hospital of Anyang City.Clinical data from 85 patients with pancreatic kininogenase(the monotherapy group)and another 85 patients with Bailing Capsule and pancreatic kininogenase(the combined group)were collected.The changes of UAER and serum biochemical index were measured to estimate the protective effect of the combined therapy.Results①There were no significant difference in male,age,triglyceride,high-density-lipoprotein-cholesterol(HDL-C),low-density-lipoprotein-cholesterol(LDL-C),body mass index(BMI),CRP and serum creatinine between groups(P>0.05).②Compared with those before theatment,the levels of 24 h urinary protein decreased after treatment showing significant difference in both groups(P<0.01).The reduction in the combined group was more significant(P<0.01).③After treatment,the levels of triglyceride and CRP decreased in both groups.There were more significant reduction in the combined group(P<0.05).④During treatment,there were only two patients with mild diarrhea in the combined group,which were relieved after ongoing treatment.Conclusion Combined therapy with Bailing Capsule and pancreatic kininogenase could significantly lower the UAER,triglyceride and CRP levels on the early-onset type 2 diabetic nephropathyⅢperiod.Ultimately,it can delay the progression of diabetic nephropathy,and without increasing adverse reactions.
