2017-2019年北京市房山区水碘及8~10岁儿童碘营养情况调查结果分析 被引量:17

Investigation on Iodine Level in Drinking Water and Iodine Nutritional Status of Children Aged 8-10 Years Old in Fangshan District of Beijing From 2017 to 2019

摘要 目的了解和掌握房山区生活饮用水水碘基线数据、8~10岁儿童碘营养及居民户盐碘状况,评估房山区现行碘缺乏病防控措施落实情况。方法采用多阶段抽样方法采集生活饮用水,检测水碘含量。抽取5个乡镇/街道地区小学,采用整群抽样方法采集8~10岁儿童家中盐样、尿样,检测碘含量,并对儿童进行甲状腺容积B超检测。结果房山区共采集248份水源水,水碘中位数为2.5μg/L,且全区水碘均<10.0μg/L;采集盐样、尿样各637份,2017-2019年盐碘中位数分别为22.0 mg/kg、28.4 mg/kg和24.5 mg/kg;碘盐覆盖率分别为89.3%、95.7%和95.6%;山区碘盐覆盖率为85.5%,合格碘盐食用率为79.5%;2017年、2018年、2019年尿碘中位数分别为193.2μg/L、201.5μg/L和164.1μg/L.结论房山区全境属于外环境缺碘地区,碘盐覆盖率达到消除标准,房山区碘缺乏病防控措施落实不到位,虽然8~10岁儿童碘营养状况良好,但仍应坚持科学补碘。 Objective To understand the baseline level of iodine in drinking water in Fangshan district and the iodine nutrition status of children aged 8-10 years old so as to evaluate the implementation of the prevention and control measures of iodine deficiency disease in Fangshan district. Methods A multi-stage sampling method was used to collect drinking water. The iodine level of the selected drinking water was detected. Five primary schools in townships/streets were selected, and the household salt and urine samples of children aged 8 to 10 years old were collected by cluster sampling method. The iodine level of those samples was detected, and the thyroid volume of children was measured by ultrasonography. Results A total of 248 pieces of source water were collected in Fangshan district. The median iodine level of those water was 2.5 μg/L,and the iodine level in those water was less than 10.0 μg/L in the whole district. In total, 637 salt and urine samples were collected. The median iodine level of salt from 2017 to 2019 was 22.0 mg/kg,28.4 mg/kg and 24.5 mg/kg,respectively. The coverage rate of iodized salt from 2017 to 2019 was89.29%, 95.71% and 95.57%, respectively. In mountainous areas, the iodized salt coverage rate was 85.54%, and the qualified iodized salt consumption rate was 79.52%. The median urinary iodine concentration of children from 2017 to 2019 was 193.2 μg/L、 201.5 μg/L and 164.1 μg/L,respectively. Conclusion The whole Fangshan district belongs to the iodine deficiency area in the outer environment. The iodine salt coverage rate reaches the elimination standard. However, the prevention and control measures of iodine deficiency disease in the mountain area still needs to be improved. Although the iodine nutritional status of children aged 8 to 10 years old is good,scientific iodine supplementation should still be adhered to.
