集束化护理理念在ICU人工气道护理中对机械通气时间及相关并发症的影响

Analysis of the influence of cluster nursing concept on the duration of mechanical ventilation and the incidence of related complications in ICU artificial airway nursing

目的探讨集束化护理理念在重症监护室(ICU)人工气道护理中对机械通气时间及相关并发症发生率的影响。方法选取2017年10月至2018年10月在佛山市南海区罗村医院ICU收治的危重症并建立人工气道患者100例,按随机数字表法分为观察组与对照组,各50例。对照组给予ICU常规护理干预,观察组在对照组基础上给予集束化护理理念,比较两组机械通气时间及相关并发症发生情况。结果观察组机械通气时间、ICU住院时间均显著短于对照组,发生肺部感染、吸入性肺炎、下肢深静脉血栓、气道阻塞及褥疮等并发症发生率(10.00%)明显低于对照组(32.00%),差异均有统计学意义(均P<0.01);入ICU时,两组APACHEⅡ评分比较差异无统计学意义;护理第7天,两组APACHEⅡ评分较入ICU时均明显降低,观察组APACHEⅡ评分显著低于对照组;观察组对护理干预的依从率(94.00%)明显高于对照组(76.00%),差异有统计学意义(P<0.05);观察组对ICU管理、护理技术、生理护理、心理护理及服务态度等各项护理工作质量评分显著高于对照组(P<0.01)。结论在ICU人工气道护理工作中采用集束化护理理念,有利于缩短患者机械通气时间及ICU住院时间,减少相关并发症的发生,帮助缓解患者病情,提高患者依从性及护理工作质量,值得临床推广实施。

Objective To explore the impact of cluster care concept on the duration of mechanical ventilation and the incidence of related complications in intensive care unit(ICU).Methods A total of 100 cases of critically ill patients admitted to the ICU of the hospital from October 2017 to October 2018 were selected and artificial airway was established.According to the random number table method,the patients were divided into the observation group and the control group,50 cases of each group.The control group was given ICU routine nursing intervention,and the observation group was given cluster nursing concept based on the control group,and the mechanical ventilation time and related complications were compared between the two groups.Results The duration of mechanical ventilation and length of stay in ICU in the observation group were significantly shorter than those in the control group,and the incidence of complications such as pulmonary infection,aspiration pneumonia,lower limb deep vein thrombosis,airway obstruction and bedsore(10.00%)was significantly lower than that of the control group(32.00%),with statistically significant differences(P<0.01).In the ICU,APACHEⅡscore was similar between the two groups,and had no statistical significance.Nursing care of seventh days,two groups of APACHEⅡscore in the ICU were significantly lower,observation group APACHEⅡscore was significantly lower than the control group.The compliance rate of nursing intervention in the observation group(94.00%)was significantly higher than that in the control group(76.00%),and the difference was statistically significant(P<0.05).The scores of ICU management,nursing technology,physiological nursing,psychological nursing and service attitude in the observation group were significantly higher than those in the control group(P<0.01).Conclusions The idea of cluster nursing in ICU artificial airway nursing is conducive to shortening patients'mechanical ventilation time and length of stay in ICU,reducing the occurrence of related complications,helping to alleviate patients'condition,improving patients'compliance and nursing quality,which is worthy of clinical promotion and implementation.
