降钙素原、C反应蛋白在小儿支气管肺炎革兰阴性阳性菌感染的初步指导意义 被引量:3

摘要 目的探讨降钙素原(PCT)、C反应蛋白(CRP)在小儿支气管肺炎革兰阴性阳性菌感染的初步指导意义。方法选取我院2017年6月至12月收治的300例支气管肺炎的患儿为研究对象。对这些病例进行回顾分析,根据痰培养结果分3组,为革兰阴性菌感染、革兰阳性菌感染及病毒感染组,分别对3组患儿的CRP、PCT、WBC、NEU等进行统计分析。结果革兰阴性菌感染组的PCT与革兰阳性菌感染及病毒感染的PCT值存在差异,P<0.05,有统计学意义;白细胞和中性粒细胞值在3组均无差异,P>0.05,无统计学意义;病毒感染组与革兰阳性菌感染组的CRP值有差异,P<0.05,有统计学意义;CRP在革兰阴性阳性菌2组中无差异,P>0.05,无统计学意义。结论CRP对于革兰阴性及阳性菌的预测价值有限,还需要较大样本量进行对比。革兰阴性菌感染的PCT值显著高于革兰阳性菌感染或病毒感染,PCT不仅可作为判断炎症病情变化的指标,对于不同类型细菌的感染也有一定的参考依据。 Objective To explore the primary significance of procalcitonin(PCT)and C-reactive protein(CRP)in children with bronchopneumonia.Methods 300 children with bronchopneumonia admitted in our hospital from June to December 2017 were selected as the study objects.According to the results of sputum culture,these cases were divided into three groups,the Gram-negative bacteria infection group,the Gram-positive bacteria infection group and the virus infection group.The CRP,PCT,WBC and NEU of the three groups were statistically analyzed.Results There was a significant difference in PCT between the Gram-negative bacteria infection group and the Gram-positive bacteria infection group(P<0.05).There was no difference in leukocyte and neutrophil values between these three groups(P>0.05),there was no statistical significance.There was a significant difference in CRP between the virus infection group and the Gram positive bacteria infection group(P<0.05).There was no difference in CRP between these two groups(P>0.05),there was no statistical significance.Conclusion CRP has a limited predictive value for Gram-negative and Gram-positive bacteria,and a large sample size is needed for comparison.The PCT value of Gram-negative bacteria infection is significantly higher than that of Gram-positive bacteria or virus infection.PCT can not only be used as an index to judge the change of inflammatory condition,but also have a certain reference for different types of bacterial infection.
