新疆维吾尔自治区区域新生儿救治中心现况调查 被引量:2

摘要 目的了解新疆维吾尔自治区区域新生儿救治中心的设备、人力资源配置现状,推动区域内新生儿救治中心建设。方法 2019年3月通过问卷调查形式对疆内16家地区级Ⅲ级甲等医院进行现况调查,对医院产科每年出生新生儿数量,新生儿病房床位数,设备,人员配备等进行调查,了解区域诊疗中心的新生儿救治基础。结果 16家Ⅲ级甲等医院,均接收新生儿病例,总分娩量36400/年,新生儿病人总床位数403张,其中6家医院床位数/分娩量比<1/100;16家医院均具备新生儿无创持续正压通气设备,15/16具备新生儿常频机械通气设备。16家医院,其中14家,新生儿独立建制。人力资源分析:有14家医院独立建制新生儿科,总计新生儿科床位数374张,新生儿科医生102人,每百张床位27名新生儿科医生;3/14的医院,各级新生儿科医生总数量小于等于5人。职称、学历、年龄分布,在不同医院之间差异较大,4家医院存在新生儿科的中级职称人员数量大于初级人员数量,25-35岁年龄组人数少于其他年龄组,提示存在新生儿科年轻人才储备不足。结论疆内地区级新生儿救治中心,逐步承担危重新生儿抢救工作,但仍处于起步阶段,存在床位数不足、设备缺乏、人力资源欠缺等突出问题。需要制定有针对性的方案,促进病房建设,学科发展,加强人员培训,提升诊疗能力。 Objective To understand the current situation of equipment and human resource allocation of newborn treatment center in Xinjiang Uygur Autonomous Region,and promote the construction of newborn treatment center in the region.Methods In March2019,a questionnaire survey was carried out in 16 prefecture level classⅢclass A hospitals in Xinjiang.The number of newborns born each year,the number of beds in the neonatal ward,equipment and staffing were investigated to understand the basis of newborn rescue in the regional diagnosis and treatment center.Results All of the 16 gradeⅢgrade A hospitals received newborn cases,with a total delivery volume of 36400/year and a total number of 403 beds for newborn patients,of which the bed/delivery volume ratio of 6 hospitals was less than 1/100;all of the 16 hospitals had non-invasive continuous positive pressure ventilation equipment for newborn,and 15/16 had constant frequency mechanical ventilation equipment for newborn.There were 16 hospitals,14 of which were independent.Human resource analysis:there were 14 hospitals independently set up newborn pediatrics,with a total of 374 beds in neonatal department,102 newborn pediatricians,27 newborn pediatricians per 100 beds;in 3/14 hospitals,the total number of newborn pediatricians at all levels were less than or equal to 5.The distribution of professional title,education background and age were quite different among different hospitals.The number of middle-level personnel with professional title were larger than that of junior personnel in four hospitals,and the number of 25-35-year-old group were smaller than that of other age groups,which suggesting that there was insufficient reserve of young talents in neonatology.Conclusion The regional newborn rescue center in Xinjiang is gradually undertaking the rescue work of critical newborn,but it is still in the initial stage,and there are some outstanding problems such as insufficient number of beds,lack of equipment,and lack of human resources.It is necessary to formulate targeted programs,promote ward construction,discipline development,strengthen personnel training and improve diagnosis and treatment capacity.
