尤文肉瘤组织LKB1和p53及Ki-67表达临床意义

摘要 目的LKB1作为新的候选抑癌基因,其在尤文肉瘤中的表达及与细胞增殖的相关性鲜见报道。本研究主要探讨LKB1、p53和Ki-67在尤文肉瘤组织中表达情况及其与临床病理指标的关系。方法采用免疫组化EnVision法检测2000-01-01-2015-06-12北京大学第三医院病理科收治的45例尤文肉瘤组织中LKB1、p53和细胞增殖指数(Ki-67)表达及其相关性,探讨其与临床病理指标的关系。结果尤文肉瘤组织中LKB1的阳性率为53.3%(24/45),强阳性表达率为13.3%(6/45)。突变型p53表达的阳性率为24.4%(11/45)。Ki-67相对高表达32例(71.1%),相对低表达13例(28.9%),LKB1表达与Ki-67呈负相关性,r_s=-0.301,P=0.044。野生型p53病例中,LKB1表达相对较高(55.9%)。LKB1及p53表达与患者年龄、性别及发病部位无统计学意义的关联性;Ki-67表达与患者年龄呈负关联性(χ^2=8.365,P=0.015),而与性别和发病部位无明显关联性。所有患者随访截至2015-10-28,获得随访27例,其中死亡17例,无瘤生存10例。结论LKB1在尤文肉瘤组织中主要呈缺失或低表达状态,其表达与Ki-67相关,提示LKB1参与了尤文肉瘤细胞生长增殖的调控,这种相关性有待扩大样本进一步研究。 OBJECTIVE LKB1,as a new candidate tumor suppressor gene,its expression and function in Ewing sarcoma have not been reported.This study aimed to explore the expression of LKB1,p53 and Ki-67 in Ewing sarcoma as well as their correlation with clinicopathological features.METHODS Immunohistochemical EnVision method was used to detect the expression of LKB1,p53 and Ki-67 in 45 cases of Ewing sarcoma from Peking University Third Hospital between January 01,2000 to June 12,2015.The correlations between the gene expression and clinicopathologic characteristics were investigated.RESULTS Positive expression of LKB1 was found in 53.3%(24/45)of Ewing sarcoma,whereas strong expression was observed in 13.3%(6/45)cases.Mutant p53 expression was detected in 24.4%cases.LKB1 expression displayed a negative correlation with Ki-67 index(r_s=-0.301,P=0.044).Cases with wild type p53 tended to present higher level of LKB1(55.9%).The expression of LKB1 and p53 was not statistically correlated with age,sex and tumor’s localization.Patients with higher Ki-67 expression tend to be younger(χ^2=8.365,P=0.015).The follow-up was until October 28,2015.Among the 45 patients,27 cases were traceable,17 cases dead and 10 cases survived.CONCLUSIONS LKB1 mainly showed negative or lower expression in Ewing sarcoma and was related to cell proliferation index,which implied a potential function in regulating proliferation in Ewing sarcoma.Large sample analyses are needed to further research.
