多节段高场强脉冲射频治疗老年胸带状疱疹后神经痛疗效观察

Observation of the treatment of senile post-herpes zoster neuralgia with multi-segment high-field pulsed radio frequency

目的观察胸背根神经节高场强脉冲射频治疗老年胸带状疱疹后神经痛(PHN)的临床疗效。方法47例确诊为胸带状疱疹后神经痛、年龄>60岁老年患者,DSA引导下给予胸背根神经节高场强脉冲射频治疗900 s,所有患者行视觉模拟评分(visual analogue scale,VAS)情况,治疗前及治疗后24 h、1周、1个月、3个月、6个月进行VAS评分,对治疗前及治疗后1个月、3个月、6个月的PSQI、SF-36、焦虑、抑郁评分进行评定,记录不良反应的发生情况。结果所有患者术后各时间点VAS评分较治疗前均明显降低(P<0.05),睡眠质量、生活质量、情绪均明显改善(P<0.05),整个治疗过程无严重并发症发生。结论高场强脉冲射频对老年胸带状疱疹后神经痛的临床疗效确切、安全。

Objective To observe the clinical effect of high field intensity pulsed radiofrequency(RF)in the treatment of post-herpes zoster neuralgia(PHN)in the elderly.Methods 47 cases were diagnosed as postherpetic neuralgia,the age was over 60 years old,the high field intensity pulsed RF was given to the thoracic dorsal root ganglion for 900 seconds under the guidance of DSA,and the visual analogue scale(VAS)was used for all the patientsbefore and after the treatment,24 hours after treatment,VAS scores were performed at 1,January,March,and June.Pre-treatment,January,March,and June after treatment The incidence of adverse reactions was recorded by PSQI,SF-36,and anxiety and depression scores.Results The VAS of the patients decreased significantly after the operation(P<0.05).The sleep quality,quality of life and the mood of the patients were significantly improved(P<0.05),and no serious complications occurred in the whole treatment.Conclusion The therapeutic effect of high-field-field pulsed radio-frequency on the treatment of postherpetic neuralgia in elderly patients with herpes zoster is accurate and safe.
