颈动脉蹼手术治疗8例临床分析

Surgical Methods for Carotid Web:a Clinical Analysis of 8 Cases

目的分析颈动脉蹼手术治疗的临床效果,为临床治疗颈动脉蹼提供参考。方法对2016年5月至2019年1月在郑州人民医院手术治疗的8例颈动脉蹼患者的临床资料进行回顾性分析,其中内膜剥脱术治疗3例,支架成形术治疗5例。结果8例患者均手术成功。内膜剥脱术治疗术后出现头痛症状1例;支架成形术治疗术中出现心动过缓1例,术后出现低血压1例。术后随访6~30个月,出现再狭窄1例,狭窄率小于30%;无靶血管重建、脑卒中或死亡病例。结论内膜剥脱术和支架成形术治疗颈动脉蹼均安全可行,如何选择手术方式应根据患者病变解剖特点、全身情况等决定。

Objective To provide reference for clinical treatment of carotid web by analyzing the clinical effects of different surgical procedures.Methods A retrospective analysis was carried out in 8 patients who were treated for carotid web in Zhengzhou People’s Hospital from May 2016 to January 2019,including 3 patients treated with carotid endarterectomy and 5 patients treated with stent-assisted angioplasty.Results All patients successfully underwent operation.Among patients treated with carotid endarterectomy,1 had headache after operation.Of patients treated with stent-assisted angioplasty,1 had bradycardia during operation,and 1 had hypotension after operation.During the follow-up of 6-30 months,restenosis occurred in 1 patient.The stenosis rate was less than 30%.No target vessel revascularization,stroke or death occurred in all patients.Conclusion Both carotid endarterectomy and stent-assisted angioplasty are safe and feasible for treating carotid web.The choice of surgical methods should be based on the anatomical characteristics of lesions and the general condition of patients.
