刘玲教授从肝脾肾辨治眩晕经验浅析

A brief analysis of Professor Liu Ling's experience in the diagnosis and treatment of vertigo from liver,spleen and kidney

本文主要介绍刘玲教授从肝脾肾三脏辨证论治眩晕的临床经验,采用的方法是从刘玲教授近一年来治疗眩晕的临床病例中提炼其治疗眩晕的经验进行理论探讨以及选取其中有代表性的几例,先辨脏腑,再辨证型,再辨方证,以求方证对应,从而治愈疾病。结果显示从肝脾肾着手辨治眩晕,多数患者取得了满意的疗效。得出结论从肝脾肾辨治眩晕临床疗效突出,方便医者临证时迅速理清思路,值得临床推广应用。目前西医对眩晕的研究日益深入,对眩晕的临床分型愈加多样,治疗方法也在不断完善,但仍有不少患者的眩晕症状得不到完全缓解,中医认为,引起眩晕的病因很多,其病机虽复杂,归纳起来不过本虚标实两个方面,本虚主要体现在肝脾肾三脏的亏虚,功能的减退,标实主要体现在风、火、痰、瘀相兼为患。刘师强调:从肝论治,要点在清肝火、息肝风、解肝郁、滋肝阴。从脾论治,要点在健脾助运,化痰祛湿。从肾论治,要点在补肾填精。对于年老体虚、久病重病之人,脏腑功能虚损,病变非在一脏一腑,治疗上当标本兼顾,脏腑之间亦可相兼为患,见肝之病,知肝传脾,此为肝脾同治。对于这类患者,用药上也需缓和,慎用峻猛之品,不求速效,以求合理缓解。

This article introduces Professor Liu Ling's experience of the dialectical treatment of liver and spleen and kidney type of vertigo.The method adopted was to extract Professor Liu Ling's experience in treating vertigo from clinical cases of vertigo in the past year and to conduct theoretical discussion and select representative cases.The zang-fu organs,dialectical type,the defense,the corresponding formula were differentiated.The results showed that the treatment of vertigo from liver,spleen and kidney was satisfactory in most patients.The conclusion is that the treatment of vertigo by liver,spleen and kidney has obvious clinical effect,and it is convenient for doctors to clarify their thoughts quickly when presenting symptoms.At present,the study of vertigo in western medicine is increasingly in-depth,clinical classification of vertigo is more and more diverse,and treatment methods are constantly improving,but there are still many patients with vertigo symptoms can not be completely relieved.Traditional Chinese medicine believes that there are many causes of vertigo.Although its pathogenesis is complex,it can be summed up in two aspects,namely,the deficiency of the liver,spleen and kidney,and the decline of function.The deficiency is mainly reflected in wind,fire,phlegm and stagnation.Professor Liu Ling's experience includes:Treatment from the liver emphasizes clearing away liver fire,extinguishing liver wind,relieving liver depression and nourishing liver Yin.Treatment from the spleen emphasizes helping transport,clearing phlegm and dampness.Treatment from the kidney emphasizes filling kidney essence.The old,weak and those who suffered from long illness,seriously ill people often show zang-fu function deficiency,the treatment should focus on both symptoms and root causes and disease prevention,the effect should be reasonable relief rather than just simple quick effect.
