加减半夏白术天麻汤联合耳穴压籽对高血压病患者合并焦虑状态的影响

Effect of Modified Banxia Baizhu Tianma Decoction and ear point pressure seed on patients with hypertension complicated with anxiety

目的:通过加减半夏白术天麻汤联合中医特色疗法“耳穴压籽”,观察对原发性高血压病患者合并焦虑状态的临床疗效。方法:选取黑龙江中医药大学附属第一医院2019年1月-2019年3月原发性高血压病合并焦虑状态患者,按随机数字表法分为,对照组60例,治疗组60例。两组患者均给予常规西药治疗及生活方式干预。治疗组在其基础上采用加减半夏白术天麻汤联合王不留行籽贴于所选耳穴。4周后观察两组患者的中医证候疗效,收缩压、舒张压水平,血压疗效,汉密尔顿焦虑量表积分。结果:中医证候疗效比较,治疗组总有效率88.33%(53/60),对照组总有效率70.00%(42/60),治疗组疗效优于对照组(P<0.05);治疗后两组内收缩压、舒张压较前明显下降,差异有显著统计学意义(P<0.05),然而在治疗后的组间比较方面,两组差异无统计学意义,但治疗组较对照组的收缩压和舒张压均有下降趋势;血压疗效比较,治疗组总有效率95.00%(57/60),对照组总有效率78.33%(47/60),治疗组疗效优于对照组(P<0.05);汉密尔顿焦虑量表积分中,治疗后,两组内积分均低于治疗前(P<0.05),在组间方面,治疗后治疗组患者汉密尔顿焦虑量表(Hamilton Anxiety Scale,HAMA)积分低于对照组(P<0.05)。结论:加减半夏白术天麻汤联合王不留行耳穴压籽对原发性高血压病患者不仅有良好的降压作用,而且可有效改善焦虑状态,无明显不良反应,值得临床推广。

Objective:To observe the effect of Modified Banxia Baizhu Tianma Decoction(半夏白术天麻汤)and ear point pressure seed on patients with hypertension complicated with anxiety.Methods:From January 2019 to March 2019,patients with primary hypertension complicated with anxiety state in the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine were selected and divided into the control group(60 cases)and the treatment group(60 cases)according to the random number table method.Patients in both groups were given conventional western medicine treatment and lifestyle intervention.The treatment group adopted Modified Banxia Baizhu Tianma Decoction and Ear Point Pressure Seed.After 4 weeks,the efficacy of TCM syndromes,systolic blood pressure,diastolic blood pressure,blood pressure effect,and Hamilton anxiety scale scores in the two groups were observed.Results:In terms of the efficacy of TCM syndromes,the total effective rate in the treatment group was 88.33%(53/60),and that of the control group was 70.00%(42/60).The therapeutic effect in the treatment group was better than that in the control group(P<0.05);After treatment,systolic blood pressure and diastolic blood pressure in the two groups were significantly lower than before,and the difference was statistically significant(P<0.05).However,in the comparison between the two groups after treatment,there was no statistical significance between the two groups,but the systolic and diastolic blood pressure in the treatment group showed a downward trend compared with that of the control group.The total effective rate in the treatment group was 95.00%(57/60),and that in the control group was 78.33%(47/60).The therapeutic effect in the treatment group was better than that in the control group(P<0.05).In the score of Hamilton anxiety scale,the internal score of both groups was lower after treatment than that before treatment(P<0.05),and the HAMA score of patients in the treatment group after treatment was lower than that in the control group(P<0.05).Conclusion:Modified Banxia Baizhu Tianma Decoction and Ear Point Pressure Seed have good antihypertensive effect on patients with primary hypertension,it also can effectively relieve the state of anxiety without obvious side effects,which is worthy of clinical promotion.
