益气活血固本方治疗特发性肺纤维化的临床观察

Clinical observation on treating idiopathic pulmonary fibrosis with the Yiqi Huoxue Guben prescription

目的:观察益气活血固本方治疗特发性肺纤维化的临床疗效。方法:将符合中西医诊断标准的患者按就诊顺序随机分为对照组与治疗组,各31例。对照组予以口服乙酰半胱氨酸颗粒,治疗组治疗在对照组基础上予以益气活血固本方治疗;均治疗6月,观察患者治疗前后肺功能指标包括用力肺活量(Forced Vital Capacity, FVC)、FVC/占预计值的百分比(FVC%)及第1秒用力呼出容积(Forced Expiratory Volume in One Second, FEV1);圣乔治呼吸问卷积分,中医证候积分及中医证候疗效。结果:与治疗前相比,两组治疗后FVC、FVC%及FEV1均下降,差异具有统计学意义(P <0.05);治疗后两组间比较,治疗组FVC、FVC%、FEV1高于对照组,差异具有统计学意义(P <0.05)。与治疗前相比,治疗组治疗后呼吸症状、疾病影响、活动受限的单项积分及总积分均下降(P <0.05),而治疗组呼吸症状、疾病影响、活动受限的单项评分及总积分升高(P <0.05)。与对照组治疗后比较,治疗组呼吸症状、疾病影响、活动受限的单项积分及总积分均显著下降(P<0.05)。与治疗前比较,治疗后主要症状积分,次要症状积分及中医证候积分均下降(P <0.05),治疗后治疗组主要症状积分,次要症状积分及中医证候积分明显低于对照组,差异具有统计学意义(P <0.05)。治疗组总有效率高于对照组,两组差异有统计学意义(P <0.05)。结论:益气活血固本方可延缓肺功能下降,改善患者呼吸症状,增加活动耐力,提高生活质量,对特发性肺纤维化临床治疗有效。

Objective: To observe the clinical efficacy of the Yiqi Huoxue Guben prescription(益气活血固本方) on idiopathic pulmonary fibrosis. Methods: Patients were randomly divided into the control group and the treatment group according to the order of treatment, 31 cases in each. The control group received acetylcysteine granules. The treatment group was treated with the Yiqi Huoxue Guben prescription more. Results: FVC, FVC% and FEV1 were more decreased in two groups after treatment than before treatment(P<0.05). And those in the treatment group were higher than those in the control group(P <0.05). The single scores and total scores of respiratory symptoms, disease effects, and activities were more decreased after treatment than before treatment(P<0.05), and those in the treatment group were better than the control group(P<0.05). The main symptom scores, secondary symptom scores and TCM syndrome scores were decreased after treatment(P<0.05). And those in the treatment group were significantly lower than the control group(P<0.05). The total efficiency in the treatment group was higher than the control group(P<0.05). Conclusion: The Yiqi Huoxue Guben prescription can delay the decline of lung function, improve respiratory symptoms, increase the activity endurance, improve the quality of life, with good efficacy.
