常州市自愿咨询检测门诊男男性行为学生人群同性一夜情的现状调查研究

One-night stand of male students who have sex with men from voluntary counseling and testing clinics in Changzhou

目的了解常州市自愿咨询检测(voluntargy counseling and testing,VCT)门诊学生男男性行为者(men who have sex with men,MSM)发生同性一夜情的现状及其影响因素,为制定艾滋病宣传教育和干预措施提供理论依据。方法选取2014年1月至2017年12月到常州市各VCT门诊主动咨询的既往有过男男性行为史的在校男性学生,开展问卷调查并采集血样进行梅毒和HIV抗体检测。结果有效问卷调查251名学生MSM,其中64.54%者近1年有过同性一夜情,HIV感染率达29.48%,梅毒感染率为5.18%。单因素分析发现:不同年龄、性取向、首个性伴性别、同性性伴数、固定性伴、同性肛交、异性性行为组别的VCT学生MSM发生同性一夜情的比例差异有统计学意义(P<0.05)。多因素非条件logistic回归分析结果显示,同性一夜情的主要影响因素有年龄>20岁(OR=2.70,95%CI=1.44~5.08)、双性恋或异性恋(OR=0.32,95%CI=0.17~0.61)、同性性伴数≥2个(OR=8.01,95%CI=3.17~20.23)、有固定性伴(OR=0.31,95%CI=0.15~0.61)。结论VCT学生MSM发生同性一夜情的现象比较普遍,年龄>20岁、同性性伴数≥2个是发生同性一夜情的危险因素,双性恋或异性恋、有固定性伴是保护因素。

Objective To understand the current status of one-night stand of men who have sex with men(MSM)from voluntary counseling and testing(VCT)clinics in Changzhou city,and to provide evidence for AIDS intervention.Methods From January 2014 to December 2017,structured questionnaire-based interviews were conducted among male students who had sex with men in VCT clinics.Blood samples were collected for HIV and syphilis antibody detection.Results Among 251 student MSM,64.54%claimed of having one-night stand with men in the most recent year.The infection rates of HIV and syphilis were 29.48%and 5.18%,respectively.Single factor analysis showed that the percentage of male student who had one-night stand with men differed significantly with age,sexual orientation,gender of first sex partner,number of homosexual partners,regular sexual partners,anal intercourse behaviors with men and heterosexual sex behaviors(P<0.05).Multivariate non-conditional logistic regression analysis showed that the factors influencing one-night stand with men were age over 20(OR=2.70,95%CI:1.44-5.08),bisexuality or heterosexuality(OR=0.32,95%CI:0.17-0.61),having 2 or more homosexual partners(OR=8.01,95%CI:3.17-20.23)and having regular homosexual partners(OR=0.31,95%CI:0.15-0.61).Conclusions One-night stand with men is common among student MSM from VCT clinics.Male students aged 20 and above,having 2 or more homosexual partners are more likely to have one night stand with men,while students with bisexuality or heterosexuality or having regular sexual partners are not.
