摘要
目的探讨血清微小RNA-144(miRNA-144)在结核病(TB)中的临床意义。方法通过计算机检索中国知网、PubMed、维普、万方、Web of science等数据库,查找建库至今国内外关于TB患者血清miRNA-144水平的病例对照研究文献。由2名作者独立检索并根据纳入及排除标准仔细阅读全文后进行选取剔除、提取数据,采用软件Stata 12.0和RevMan5.3进行统计分析。结果本研究共纳入9篇文献,包括TB患者431例(病例组),健康者354例(健康组)。荟萃分析结果显示总体人群中病例组血清miRNA-144水平较健康组升高(SMD=3.08,95%CI:1.26~4.90,P<0.05)。根据TB的不同类型进行亚组分析显示,病例组血清miRNA-144水平较健康组升高(SMD=2.26,95%CI:0.62~3.90,P<0.05)。根据人种差异进行分析,黄种人病例组血清miRNA-144水平较健康组升高(SMD=3.46,95%CI:1.34~5.58,P<0.05)。根据不同年龄进行亚组分析,以12岁为分界,≥12岁病例组血清miRNA-144水平较健康组升高(SMD=2.78,95%CI:0.88~4.68,P<0.05)。结论血清miRNA-144水平可能与TB具有一定相关性,尤其在黄种人TB中水平较高。但由于纳入研究中样本量较少,需更多标准化、样本量大、同质性好的研究进一步验证。
Objective To investigate the clinical significance of serum microRNA-144(miRNA-144)in tuberculosis(TB).Methods Case-control studies on the association between serum miRNA-144 level and tuberculosis were searched in the databases,including CNKI,Wanfang,Vip,PubMed,Web of science from the time of the establishment of the databases to now.The two evaluators independently screened the literature according to the inclusion criteria and exclusion criteria,and extracted the data.Stata 12.0 and RevMan5.3 software were used to evaluate the meta-analysis.Results A total of 9 articles were included in this study,including 431 TB patients(case group)and 354 healthy patients(healthy group).The results of meta-analysis indicated that serum miRNA-144 levels in the case group were higher in the general population than in the healthy group(SMD=3.08,95%CI:1.26-4.90,P<0.05).Subgroup analysis based on different types of TB showed that serum miRNA-144 level in the case group was higher than that in the healthy group(SMD=2.26,95%CI:0.62-3.90,P<0.05).According to the analysis of ethnic differences,the serum miRNA-144 level in the yellow Caucasian group was higher than that in the healthy group(SMD=3.46,95%CI:1.34-5.58,P<0.05).Subgroup analysis was performed according to different ages,with 12 years as the dividing line.Serum miRNA-144 level in the case group≥12 years old was higher than that in the healthy group(SMD=2.78,95%CI:0.88-4.68,P<0.05).Conclusions Serum miRNA-144 levels may be correlated with TB,especially in the yellow race.However,due to the small sample size in the included studies,the conclusion needs to be verified bymore standardized,large sampleswith good homogeneityin the future.
