食管裂孔疝中医舌象与胃镜像关系研究

Study on the relationship between tongue image and gastroscope image of esophageal hiatal hernia

目的:观察食管裂孔疝的不同中医临床证型舌象与内镜像之间的相关性。方法:对62例食管裂孔疝患者的舌象及胃镜像进行分析、观察。结果:62例食管裂孔疝患者中,不同舌色组的食管反流、胆汁反流程度整体比较各组无统计学差异;不同舌苔组的食管反流程度比较各组之间有统计学差异(P﹤0.05),食管反流和舌苔具有相关性,其中薄白苔组与黄腻苔组有统计学差异(P﹤0.05),spearaman相关分析,r=0.571,P=0.00﹤0.05,表明两组呈中度正相关性;其余各组间两两比较,均无统计学差异,组间无相关性。不同舌苔各组间胆汁反流比较,有统计学差异(P﹤0.05)。结论:食管裂孔疝合并反流性食管炎、胆汁反流的不同中医临床证型舌象与内镜像之间存在一定的相关性,为中医辨证提供新的客观化指标,提高了对食管裂孔疝中医辨证治疗规律的认识。

Objeetive:To explore the correlation between the tongue image of different TCM clinical syndromes and the internal image of esophageal hiatal hernia.Method:The tongue image and the internal image in 62 cases of esophageal hiatal hernia were observed and analyzed.Result:In 62 cases of esophageal hiatal hernia,there was no difference in the degree of esophageal and bile reflux in different tongue color groups,and there was difference in the degree of esophageal reflux in different furred tongue groups(P﹤0.05),indicating that there was correlation between esophageal reflux and furred tongue.There was a significant difference between thin white fur group and greasy fur group(P﹤0.05).The correlation analysis of speraman was r=0.571,P=0.00﹤0.05,indicating that the two groups had a moderate positive correlation.Pairwise comparisons were made between the other groups and there was no statistical difference,indicating that there was no correlation between the other groups.There were differences in bile reflux between different furred tongue groups(P﹤0.05).Conclusion:Different TCM clinical syndromes of esophageal hiatal hernia with reflux esophagitis and bile reflux have certain correlation with internal image,which provides a new objective index for TCM syndrome differentiation,improves the understanding of TCM syndrome differentiation and treatment law of esophageal hiatal hernia,provides an objective basis for TCM treatment of esophageal hiatal hernia,has a positive significance for the development of TCM and the integration of Chinese and Western medicine,and promotes the scientificization and standardization of esophageal hiatal hernia At the same time,it further confirmed the advantages of TCM syndrome differentiation and treatment.
