固定-活动义齿联合修复老年牙周病伴牙列缺损的近期随访分析

Short-term follow-up on outcomes of hybrid repair with fixed-removable denture for dentition defect in elderly with periodontal disorder

目的:探讨固定-活动义齿联合修复老年牙周病伴牙列缺损的近期疗效。方法:选取2015年1月至2017年1月我院收治的160例老年牙周病伴牙列缺损患者为研究对象,按照数字表法将其分为观察组(80例)和对照组(80例),给予观察组固定-活动义齿联合修复治疗,给予对照组活动义齿修复治疗,随访18个月,比较两组修复效果、患者口腔健康相关生活质量、牙龈出血指数及基牙拔除情况。结果:观察组患者牙周病伴牙列缺损的临床治疗总有效率高于对照组,但组间比较差异无统计学意义(P>0.05);修复后18个月,观察组患者的OHIP-14评分及牙龈出血指数均明显低于对照组(P<0.05);观察组修复治疗后的基牙拔除率明显低于对照组(P<0.05)。结论:固定-活动义齿联合修复老年牙周病伴牙列缺损的近期疗效肯定,利于患者口腔健康状况及生活质量的改善,值得推广运用。

Objective:To investigate the short-term outcomes of hybrid repair with fixed-removable denture for dentition defect in elderly with periodontal disorder.Methods:A total of 160 elderly patients with periodontal disorder and dentition defect admitted to our hospital between January 2015 and January 2017 were included as subjects,divided into the study group(n=80)and control group(n=80)according to random number table.The study group received hybrid repair with fixed-movable denture,and the control group received repair with removable denture.The two groups were followed up for 18 months.The repair outcomes,oral health related quality of life,gingival bleeding index and removal of dental abutment were compared between the two groups.Results:The total effective rate for clinical treatment of periodontal disorder with dentition defect in the study group was higher than that in control group,but the difference did not reach the level of statistical significance(P>0.05).At 18 months after repair,the OHIP-14 score and gingival bleeding index in the study group were significantly lower than those in the control group(P<0.05).The rate of abutment removal in the study group was significantly lower than that in the control group(P<0.05).Conclusion:Hybrid repair with fixed-movable denture for elderly patients with periodontal disorder and dentition defect offers a definite favorable short-term outcome which is conducive to oral heath and quality of life,and therefore is justified for widespread use.
