腹膜假性黏液瘤的MSCT影像学特征分析

Analysis of MSCT imaging features of pseudomyxoma peritonei

目的探讨腹膜假性黏液瘤(PMP)的多排螺旋CT(MSCT)表现,提高PMP的诊断水平。方法回顾性分析8例经临床术后病理证实的PMP患者的MSCT表现、临床资料及其病理学特点。结果8例PMP的CT平扫主要表现为腹盆腔多发囊性或稍低密度结节黏液肿块,尤以膈下、网膜及肠系膜分布明显,边界不清,密度不均,增强扫描病变呈轻度延迟强化,肿块对腹盆腔内脏器表面挤压明显可见典型“扇贝样”压迹;腹盆腔内不同程度积液,位置固定;术前CT检查发现原发肿瘤灶3例,出现脾脏转移1例。结论PMP临床上非常罕见,MSCT可充分显示病变的影像学特点,对疾病的正确诊断及随访、复查有重要意义。

Objective To investigate the MSCT imaging features findings of pseudomyxoma peritonei(PMP)and improve the diagnostic level of it.Methods Retrospective analysis was carried out on the MSCT findings,clinical data and pathological features of 8 patients with PMP by postoperative pathology.Results CT plain scan of 8 cases of PMP mainly showed multiple cystic or slightly low-density nodule mucinous masses in the abdominal and pelvic cavity,especially in the subphrenic,omental and mesenteric areas with obvious distribution,unclear boundary and uneven density.On contrast-enhanced scan,the lesions showed mild delayed enhancement,and the typical"scallop-shape"impression could be seen on the surface of abdominal and pelvic visceral organs.There were different degrees of effusion in the abdominal cavity or pelvic cavity,and the positions were fixed.3 cases of primary tumor and 1 case of spleen metastasis were found by CT examination.Conclusion Pseudomyxoma peritonei is very rare in clinic.MSCT examination can fully display the imaging features of the lesion,which is of great significance for the correct diagnosis,follow-up and review of the disease.
