上颌窦内提升术在口腔种植修复治疗中的疗效观察

Effect of Maxillary Sinus Augmentation on Oral Implant Succession and Periodontal Health Status in Oral Implant Restoration

目的探讨口腔种植修复中采用上颌窦内提升术对种植体成功率及牙周健康指标的影响。方法选取2014年6月—2016年1月山东省青岛疗养院收治的上颌前磨牙或磨牙缺损实施口腔种植修复治疗的患者80例进行回顾性分析。根据治疗方法分为A组40例(采用上颌窦内提升术实施种植修复)、B组40例(采用常规种植修复技术)。对比两组术后1年、2年种植体留存率、术后不同时间点种植体周围骨吸收量、牙周菌斑指数、探诊深度和龈沟出血指数。结果术后1个月、术后6个月和术后12个月A组患者的种植体周围骨吸收量显著的高于B组;术后24个月,A组和B组患者的种植体周围骨吸收量差异不具有统计学意义;术后6个月,A组患者的牙周菌斑指数、探诊深度和龈沟出血指数均显著低于B组;术后1年,A组和B组患者的种植体留存率比较,差异不具有统计学意义;术后2年,A组患者的种植体留存率为96.00%,高于B组患者的83.02%。结论口腔种植修复中采用上颌窦内提升术有利于促进种植体周围骨吸收、提高种植体的留存率、降低种植修复对牙周健康的损害程度。

OBJECTIVE To explore the effect of maxillary sinus lifting on implant success rate and periodontal health index in oral implant restoration.METHODS A retrospective analysis was performed on 80 patients who underwent oral implant restoration in the maxillary premolars or molar defects admitted to target hospital from June 2014 to January 2016.According to the treatment method,40 patients in group A(planting repair with maxillary sinus lift)and 40 patients in group B(were treated with conventional implant repair techniques).The implant retention rate at 1 year and 2 years after operation,bone resorption around the implant at different time points after surgery,periodontal plaque index,depth of probing,and sulcus bleeding index were compared.RESULTS The bone resorption around the implants in group A was significantly higher than that in group B at 1 month,6 months after surgery and 12 months after surgery.At 24 months after operation,there was no significant difference in bone resorption between implants in group A and group B.At 6 months after operation,the periodontal plaque index,depth of probing,and sulcus bleeding index of group A were significantly lower than those of group B.One year after operation,the retention rate of implants in group A and group B was not statistically significant.At 2 years after surgery,the implant retention rate of group A was 96.00% higher than group B(83.02%).CONCLUSION The use of maxillary sinus lifting in oral implant restoration is beneficial to promote bone resorption around the implant,improve the retention rate of the implant,and reduce the damage of implant restoration to periodontal health.
