全髋与半髋关节置换术治疗老年人股骨颈骨折结果及安全性对比

Comparison of the results and safety of total hip arthroplasty and hemiarthroplasty in the treatment of femoral neck fractures in the elderly

目的 探究全髋与半髋关节置换术治疗老年人股骨颈骨折结果及安全性对比.方法 选取我院2017年3月~2019年6月收治的老年股骨颈骨折患者102例,根据不同的治疗方法分为对照组及研究组,各51例.对照组采用半髋关节置换术,研究组采用全髋关节置换术,比较两组临床疗效、出血量、手术时间以及社会功能、生理功能、情感职能,并进行分析.结果 研究组患者疗效高于对照组,差异有统计学意义(P<0.05);手术后两组患者社会功能情况、生理功能、情感职能均明显升高,差异有统计学意义(P<0.05),且研究组高于对照组,差异有统计学意义(P<0.05);研究组患者手术出血量、手术时间均高于对照组,差异有统计学意义(P<0.05).结论 全髋关节置换术与半髋关节置换术相比较,全髋关节置换术的临床疗效更好,虽然手术时间长,出血量较多,但是术后恢复快,值得临床广泛推广及使用.

Objective To explore the results and safety of total hip arthroplasty and hemiarthroplasty in the treatment of femoral neck fractures in the elderly.Methods 102 elderly patients with femoral neck fractures treated in our hospital from March 2017 to June 2019 were selected and divided into a control group and a study group,with 51 cases in each according to different treatment methods.The control group was treated with hemiarthroplasty,and the study group was treated with total hip arthroplasty.The clinical efficacy,bleeding volume,operation time,social function,physiological function,and emotional function of the two groups were compared and analyzed.Results The efficacy of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).The social function,physiological function,and emotional function of the two groups significantly increased after surgery,as compared with those before surgery,and the differences were statistically significant(P<0.05).And the study group was higher than the control group,the difference was statistically significant(P<0.05).The surgical bleeding volume and operation time in the study group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Compared with hemiarthroplasty,total hip arthroplasty has better clinical effect.The total hip arthroplasty recovers fast,although it features longer operation time and more bleeding,which is worthy of wide clinical application.
