上海周浦地区尿液NMP22阳性与尿常规检查和血液检查的多因素分析

Multivariate analysis of NMP22 positive with routine urine test and blood test in Zhoupu district,Shanghai

目的回顾性分析患者尿液核基质蛋白22(NMP22)检测阳性结果与尿常规检查、血液检查指标相关情况,为膀胱癌的实验室辅助诊断提供线索和依据。方法收集上海健康医学院附属周浦医院2014年7月至2018年12月进行尿液NMP22检测的患者病历资料和实验室检查数据,将NMP22阳性的患者分为膀胱癌患者61例(肿瘤组)和泌尿系统良性疾病患者75例(对照组),运用单因素和多因素Logisitic回归分析尿常规检查、血液检查指标联合诊断膀胱癌的价值。结果多因素Logisitic回归分析显示年龄、尿常规中的尿蛋白、尿红细胞和尿白细胞强阳性,血液检测高于最佳截断值(Cut-off值)的淀粉样蛋白A(SAA)和铁蛋白(SF)是影响膀胱癌患者NMP22阳性的危险因素。对应的OR值分别是2.989、3.663、4.212、2.681、3.050、2.933,P值分别是0.017、0.004、0.008、0.032、0.010、0.015。结论NMP22阳性与膀胱癌患者年龄、尿常规中尿蛋白、尿红细胞、尿白细胞强阳性,血液检测高于Cut-off值的SAA和SF等多种因素相关;NMP22检测结合尿常规检查、血液检查指标在鉴别膀胱癌与泌尿系统良性疾病中具有重要意义。

Objective To retrospectively analyze the situation of the patients of positive result of nuclear matrix protein 22(NMP22),and related conditions of detection of routine urine and blood parameters test,To provide clues and evidences for diagnosis of bladder cancer.Methods The medical record data of patients who were detected on NMP22-positive were collected from July 2014 to December 2018 in Affiliated Zhoupu Hospital of Shanghai University of Medicine and Health Sciences.There were 61 patients with bladder cancer(bladder cancer group),75 patients with benign diseases of the urinary system(disease control group).The univariate analysis and Logistic regression analysis were used to evaluate the roles of routine urine test and blood test in the combined diagnosis of bladder cancer.Results Logistic regression showed that the ages of patients,strong positive of proteinuria,urinary erythrocyte and urinary leukocyte in routine urine,exceed cut-off value of serum amyloid A(SAA)and ferritin(SF)were the relevant hazards factors to NMP22-positive of the patients with bladder cancer.Which the corresponding odd ration(OR)were 2.989,3.663,4.212,2.681,3.050,2.933;And corresponding P-value were 0.017,0.004,0.008,0.032,0.010,0.015,respectively.Conclusion The NMP22-positive of bladder cancer group may be correlated with many factors such as the ages of patients,strong positive of proteinuria,urinary erythrocyte and urinary leukocyte in routine urine,exceed cut-off value of CRP,SAA and SF.Detection of NMP22 combined with routine urinalysis and blood tests had important significance to distinguish between bladder cancer and benign diseases of the urinary system.
