摘要
目的分析肺表面活性蛋白(SP-D)与甘露聚糖结合凝集素(MBL)在巨细胞病毒宫内感染表达的临床价值。方法选择2012年8月-2017年11月新疆医科大学第一附属医院产科门诊进行孕中期产前检查的孕妇,因其他因素需进行羊水穿刺,具备羊水穿刺指征的患者1291例作为研究对象,分别检测其血清人巨细胞病毒(HCMV-IgM)、血清HCMV-DNA,羊水HCMV-DNA,以及血清中SP-D与MBL表达水平。结果1291例孕妇中有HCMV感染孕妇46例。46例HCMV感染孕妇中有18例为HCMV宫内感染。宫内感染组孕妇血清SP-D和MBL水平显著高于非宫内感染组,差异具有统计学意义(P<0.05)。非HCMV感染组孕妇血清SP-D和MBL水平均显著低于宫内感染组和非宫内感染组,差异具有统计学意义(P<0.05)。结论巨细胞病毒宫内感染孕妇血清SP-D与MBL水平显著上调,对于辅助临床确诊巨细胞病毒宫内感染具有一定的参考价值,可作为潜在生物标志物进行研究。
Objective To analyze the clinical value of SP-D and Mannan binding lectin(MBL)expression in cytomegalovirus(CMV)intrauterine infection.Methods Amniocentesis was performed due to some other factors,and 1291 patients with amniocentesis indications were selected as study objects who underwent antenatal examination in the second trimester of pregnancy in the hospital from August 2012 to November 2017.Serum HCMV-IgM,serum HCMV-DNA and amniotic fluid HCMV-DNA,the expression of SP-D and MBL in serum were detected,respectively.Results Forty-six cases from 1291 pregnant women were infected with HCMV.Of the 46 pregnant women with HCMV infection,18 were HCMV intrauterine infections.The serum SP-D and MBL levels of pregnant women with intrauterine infection were significantly higher than those of pregnant women without intrauterine infection,with significant difference(P<0.05).The serum SP-D and MBL levels of pregnant women without HCMV infection were significantly lower than those of intrauterine infection group and non-intrauterine infection group(P<0.05).Conclusion The serum SP-D and MBL levels of pregnant women with intrauterine cytomegalovirus infection were significantly up-regulated.It has certain reference value for assistant clinical diagnosis of cytomegalovirus intrauterine infection and can be used as a potential biomarker to investigate.
