腹腔镜下解剖性肝切除联合腹横纹肌阻滞治疗原发性肝癌的疗效及对患者相关血清指标和预后的影响

Effect of Laparoscopic Anatomical Hepatectomy combined with Abdominal Rhabdomyoblasty on Patients with Primary Hepatocellular Carcinoma and Its Related Serum Indicators and Prognosis

目的探讨腹腔镜下解剖性肝切除术(laparoscope precise hepatectomy,LPH)联合腹横纹肌阻滞(transversus abdominis plane bloc,TAPB)治疗原发性肝癌(PLC)的疗效及对患者相关血清指标和预后的影响。方法选取100例PLC患者,根据治疗方式的不同,分为研究组及对照组。研究组50例,采用LPH联合TAPB治疗;对照组50例,采用腹腔镜下非解剖性肝切除治疗。比较2组患者的临床疗效等差异。结果研究组除手术时间长于对照组外,其余围术期相关指标均显著优于对照组(P<0.05)。研究组的麻醉相关指标均显著优于对照组患者(P<0.05)。术前2组患者的相关血清指标比较无差异(P>0.05),而术后研究组患者的相关血清指标均较对照组改善明显(P<0.05)。术后1年,2组患者的复发或转移率比较,差异无统计学意义(P>0.05);而研究组患者的生存率(74%)显著高于对照组患者(54%)(P<0.05)。结论LPH联合TAPB治疗PLC的疗效显著,不仅术后镇痛效果较佳,而且具有免疫功能保护作用,并可改善患者预后,值得临床推广应用。

Objective To investigate the analysis of Clinical efficacy、serum indexes and prognosis of application of LPH and TAPB in the treatment of patients with PLC.Methods To screen 100 cases of PLC patients as research subjects,it will be divided into the study group with 50 cases and the control group with 50 cases,patients in the control group were treated with laparoscopic non-anatomical hepatectomy and patients in the study group were treated with LPH and TAPB,then the 2 groups were compared in terms of clinical efficacy and so on.Results The clinical indexes in the study group were better than the control group except the operation time(P<0.05);The analgesia indexes in the study group were better than the control group after treatment(P<0.05);The serum indexes in the study group were better than the control group after treatment(P<0.05);The prognosis in the study group was better than the control group after treatment(P<0.05).Conclusion The LPH and TAPB in the treatment of patients with PLC is more significant,it is worthy of popularization and application.
