湿润烧伤膏联合银离子抗菌敷料对Ⅱ度烧伤患者血清炎症因子的影响

Influence of MEBO Combined with Silver Ion Antibacterial Dressing on Serum Inflammatory Factors in Patients with Second Degree Burns

目的探讨湿润烧伤膏联合银离子抗菌敷料对Ⅱ度烧伤患者血清炎症因子的影响。方法选取2015年5月至2019年5月长沙医学院附属株洲市人民医院收治的68例Ⅱ度烧伤患者作为研究对象,并按照随机数表法将其随机分为试验组(34例)和对照组(34例),其中试验组患者予以湿润烧伤膏联合银离子抗菌敷料治疗局部创面,对照组患者单纯予以湿润烧伤膏治疗局部创面,对比观察两组患者血清炎症因子白细胞介素-6(IL-6)、白细胞介素-8 (IL-8)、肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)水平以及临床疗效。结果治疗第7、14天,两组患者血清IL-6、IL-8、TNF-α和hs-CRP水平均明显低于治疗前(试验组:F=42.130、57.090、37.450、63.660,P均=0.000;对照组:F=12.230、32.130、22.300、37.950,P均=0.000),且试验组明显低于对照组(治疗第7天:t=3.016、2.657、3.887、5.037,P=0.004、0.010、0.000、0.000;治疗第14天:t=7.004、4.074、4.511、6.589,P均=0.000)。治疗3周后,试验组患者痊愈率为88.24%,明显高于对照组患者痊愈率64.71%(Mann-Whitney U=442.000,Z=-2.270,P=0.023)。结论湿润烧伤膏联合银离子抗菌敷料可有效降低Ⅱ度烧伤患者血清炎症因子水平,减轻创面炎症反应,具有协同降低机体炎症反应程度和促进创面愈合的作用。

Objective To explore the influence of MEBO combined with silver ion antibacterial dressing on serum inflammatory factors in patients with second degree burns.Methods Sixty-eight patients with second degree burns,admitted to Zhuzhou People’s Hospital Affiliated to Changsha Medical University from May 2015 to May 2019,were selected as the research subjects,and then divided,according to random number table,into an experiment group(34 cases) and a control group(34 cases).Patients in the experiment group were treated with MEBO combined with silver ion antibacterial dressings while patients in the control group were treated with MEBO only.The levels of serum inflammatory factors,interleukin-6(IL-6),interleukin-8(IL-8),tumor necrosis factor-α(TNF-α),high-sensitivity C-reactive protein(hs-CRP) and therapeutic effect were compared between the two groups.Results The levels of serum IL-6,IL-8,TNF-αand hs-CRP in the two groups on day 7 and day 14 of treatment were significantly lower than before treatment(Experiment group:F=42.130,57.090,37.450,63.660,all P=0.000;control group:F=12.230,32.130,22.300,37.950,all P=0.000),and their levels in the experiment group was significantly lower than the control group(day 7 of treatment:t=3.016,2.657,3.887,5.037,P=0.004,0.010,0.000,0.000;day 14 of treatment:t=7.004,4.074,4.511,6.589,all P=0.000) After 3 weeks of treatment,the wound healing rate in the experiment group was 88.24%,which was significantly higher than the control group-64.71%(Mann-Whitney U=442.000,Z=-2.270,P=0.023).Conclusion MEBO combined with silver ion antibacterial dressing can reduce the levels of serum inflammatory factors and inflammatory reaction on the wounds of patients with second degree burns.It can synergistically weaken inflammatory reaction and promote wound healing.
