吸烟对稳定期COPD患者Th17/Treg细胞表达的影响及其与血清炎性因子的相关性分析

Effect of smoking on Th17/Treg cell expression in stable COPD patients and its correlation with serum inflammatory factors

目的研究吸烟对稳定期慢性阻塞性肺疾病(COPD)患者辅助性T细胞17/调节性T细胞(Th17/Treg)细胞表达的影响及其与血清炎性因子的相关性。方法选取50例稳定期COPD患者作为观察组,根据患者吸烟情况分为吸烟组(15例)、戒烟组(20例)、非吸烟组(15例);另选取同期体检健康志愿者50例作为对照组。检测并对比两组肺功能指标及实验室指标[第1秒用力呼气容积(FEV1)、第1秒用力呼气容积占用力肺活量比例(FEV1/FVC)、辅助性T细胞17/调节性T细胞(Th17/Treg)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-8(IL-8)],对比吸烟组、戒烟组、非吸烟组患者FEV1、FEV1/FVC、Th17/Treg、TNF-α、IL-6、IL-8水平,采用Pearson相关系数分析FEV1、FEV1/FVC与Th17/Treg、TNF-α、IL-6、IL-8及Th17/Treg与TNF-α、IL-6、IL-8的相关性。结果观察组FEV1(42.59±7.52)%、FEV1/FVC(43.24±8.26)%均显著低于对照组的(89.40±10.43)%、(90.62±10.33)%,差异有统计学意义(P<0.05);观察组Th17/Treg(3.11±0.54)%、TNF-α(14.25±6.32)ng/L、IL-6(19.52±8.98)ng/L、IL-8(78.52±7.85)ng/L均显著高于对照组的(1.06±0.21)%、(5.34±2.40)ng/L、(8.85±5.95)ng/L、(62.43±3.24)ng/L,差异有统计学意义(P<0.05)。吸烟组、戒烟组、非吸烟组患者的FEV1、FEV1/FVC水平呈升高趋势,组间比较差异具有统计学意义(P<0.05);吸烟组、戒烟组、非吸烟组患者的Th17/Treg、TNF-α、IL-6、IL-8水平呈下降趋势,组间比较差异具有统计学意义(P<0.05)。经相关性分析,FEV1、FEV1/FVC与Th17/Treg、TNF-α、IL-6、IL-8呈负相关(P<0.05)。经相关性分析,Th17/Treg与TNF-α、IL-6、IL-8呈正相关(r=0.547、0.526、0.634,P=0.000、0.000、0.000<0.05)。结论稳定期COPD患者Th17/Treg、TNF-α、IL-6、IL-8水平呈现出高表达,且Th17/Treg、TNF-α、IL-6、IL-8水平的上升与吸烟密切相关,患者应当及早戒烟。

Objective To study the effect of smoking on helper T cells 17/regulatory T cells(Th17/Treg)cell expression in stable chronic obstructive pulmonary disease(COPD)patients and its correlation with serum inflammatory factors.Methods A total of 50 stable COPD patients were selected as the observation group,which were divided into smoking group(15 cases),smoking cessation group(20 cases)and non-smoking group(15 cases)by smoking status.Another 50 healthy volunteers were selected as the control group.The pulmonary function indexes and laboratory indexes[forced expiratory volume in 1st second(FEV1),FEV1/force vital capacity(FVC),Th17/Treg,tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-8(IL-8)]were detected and compared between the two groups.The levels of FEV1,FEV1/FVC,Th17/Treg,TNF-α,IL-6 and IL-8 in smoking group,smoking cessation group and non-smoking group were compared.Pearson correlation coefficient was used to analyze the correlation between FEV1,FEV1/FVC and Th17/Treg,TNF-α,IL-6,IL-8,Th17/Treg and TNF-α,IL-6,IL-8.Results FEV1(42.59±7.52)%and FEV1/FVC(43.24±8.26)%of the observation group were significantly lower than those of the control group(89.40±10.43)%and(90.62±10.33)%,and the difference was statistically significant(P<0.05).Th17/Treg(3.11±0.54)%,TNF-α(14.25±6.32)ng/L,IL-6(19.52±8.98)ng/L and IL-8(78.52±7.85)ng/L of the observation group were significantly higher than those of the control group(1.06±0.21)%,(5.34±2.40)ng/L,(8.85±5.95)ng/L and(62.43±3.24)ng/L,and the difference was statistically significant(P<0.05).The levels of FEV1 and FEV1/FVC of smoking group,smoking cessation group and non-smoking group showed an increasing trend,and the difference between the groups was statistically significant(P<0.05).The levels of Th17/Treg,TNF-α,IL-6 and IL-8 in the smoking group,smoking cessation group and non-smoking group showed a downward trend,and the difference between the groups was statistically significant(P<0.05).By correlation analysis,FEV1 and FEV1/FVC were negatively correlated with Th17/Treg,TNF-α,IL-6 and IL-8(P<0.05).Through correlation analysis,Th17/Treg was positively correlated with TNF-α,IL-6,IL-8(r=0.547,0.526,0.634,P=0.000,0.000,0.000<0.05).Conclusion Th17/Treg,TNF-α,IL-6 and IL-8 levels are highly expressed in stable COPD patients,and the increase of Th17/Treg,TNF-α,IL-6 and IL-8 levels is closely related to smoking,so patients should give up smoking as soon as possible.
