血清β⁃hCG和孕酮表达与妊娠剧吐患者甲状腺功能 和胃动素相关性 被引量:11

Correlation of serumβ⁃hCG and progesterone expression with thyroid function and motilin in patients with hyperemesis gravidarum

摘要 目的探究妊娠剧吐患者血清β⁃人绒毛膜促性腺激素(β⁃hCG)、孕酮(P)表达及与甲状腺功能、胃动素(MOT)的相关性。方法选取2018年1月至2019年10月本院收治的81例妊娠剧吐孕妇作为研究组,选取79例正常孕妇作为对照组。检测比较两组血清β⁃hCG、P、MOT水平、妊娠剧吐程度(VAS评分)、甲状腺功能[游离甲状腺素(FT4)、游离三碘甲状腺原氨酸(FT3)、促甲状腺素(TSH)],分析血清β⁃hCG、P水平与VAS评分、甲状腺功能、MOT的相关性,探讨血清β⁃hCG、P与妊娠剧吐的关系。结果研究组血清β⁃hCG水平高于对照组,血清P水平低于对照组(P<0.05);研究组血清FT3、FT4水平及VAS评分高于对照组,血清TSH及MOT水平低于对照组(P<0.05);血清β⁃hCG水平与血清TSH、MOT水平存在显著负相关,与血清FT3、FT4水平及VAS评分存在显著正相关(P<0.05);血清P水平与血清TSH、MOT水平存在正相关,与血清FT3、FT4水平及VAS评分存在显著负相关(P<0.05);血清β⁃hCG、P与妊娠剧吐高度相关(P<0.05)。结论妊娠剧吐患者血清β⁃hCG表达异常升高,P表达较低,与患者甲状腺功能、MOT水平及妊娠剧吐程度关系密切,监测其变化情况可为临床合理制定干预措施提供指导性参考。 Objective To investigate the expression of serumβ⁃human chorionic gonadotropin(β⁃hCG)and progesterone(P)in patients with hyperemesis gravidarum and the correlation with thyroid function and motilin(MOT).Methods From January 2018 to October 2019,81 pregnant women with severe vomit⁃ing in hospital were selected as the study group and 79 normal pregnant women were selected as the control group.The serumβ⁃hCG,P,MOT levels,hyperemesis gravidarum(VAS score),thyroid function[free thy⁃roxine(FT4),free triiodothyronine(FT3),and thyroid stimulating hormone(TSH)]were measured and compared between the two groups.The correlations of serumβ⁃hCG and P levels with VAS score and thyroid function and MOT were analyzed.Results The serumβ⁃hCG level in the study group was higher than that in the control group,and the serum P level was lower than that in the control group(P<0.05).Serum FT3 and FT4 levels and VAS scores in the study group were higher than those in the control group,and serum TSH and MOT levels were lower than those in the control group(P<0.05).There was a significant negative correlation between serumβ⁃hCG levels and serum TSH and MOT levels,and a significant positive correlation with serum FT3,FT4 levels,and VAS scores(P<0.05).Serum P level was positively correlated with serum TSH and MOT levels,and negatively correlated with serum FT3,FT4 levels and VAS scores(P<0.05).Serumβ⁃hCG and P were highly related to hyperemesis gravidarum(P<0.05).Conclusion Serumβ⁃hCG expression is abnormally increased in patients with hyperemesis gravidarum,and the expression of P is low,which is closely related to the patients􀆳thyroid function,MOT level and the degree of hyperemesis gravidarum.Moni⁃toring the changes can provide a guidance for the clinical rational intervention.
