宫腔镜下子宫内膜息肉电切术治疗子宫内膜息肉的临床疗效

Clinical effect of hysteroscopic transcervical electrocision of polyps in patients with endometrial polyps

目的探讨宫腔镜下子宫内膜息肉电切术治疗子宫内膜息肉的临床疗效。方法84例无生育要求的子宫内膜息肉患者,采用随机数字表法分为对照组和研究组,各42例。对照组患者接受宫腔镜下刮宫术治疗,研究组患者接受宫腔镜下子宫内膜息肉电切术。比较两组患者手术时间、出血量以及住院时间,术前和术后3、6个月月经量变化情况,术后并发症发生情况以及复发率。结果研究组患者手术时间(12.1±2.3)min、出血量(4.2±1.2)ml以及住院时间(3.5±1.1)d与对照组的(11.5±2.1)min、(3.8±2.1)ml、(3.2±1.2)d比较,差异无统计学意义(P>0.05)。研究组术后3、6个月的月经量分别为(128.2±13.4)、(153.4±13.4)ml,均优于对照组的(257.1±12.3)、(305.1±23.4)ml,差异具有统计学意义(P<0.05)。研究组术后并发症发生率7.14%、复发率4.76%均低于对照组的30.95%、19.05%,差异均具有统计学意义(P<0.05)。结论对于子宫内膜息肉患者采用宫腔镜下子宫内膜息肉电切术出血量少,住院时间短,并发症少,并且可以有效降低患者疾病复发率,减少经量,值得临床推广应用。

Objective To discuss the clinical efficacy of hysteroscopic transcervical electrocision of polyps in patients with endometrial polyps.Methods A total of 84 endometrial polyps without fertility requirements were divided into control group and research group by random number table method,with 42 cases in each group.The control group was treated by hysteroscopic dilatation and curettage,and the research group was treated by hysteroscopic transcervical electrocision.The surgery time,amount of hemorrhage,hospitalization time,menstrual volume changes before before surgery,3 and 6 months after surgery,occurrence of postoperative complications and recurrence rate were compared between the two groups.Results The surgery time(12.1±2.3)min,amount of hemorrhage(4.2±1.2)ml and hospitalization time(3.5±1.1)d of the research group had no statistically significant difference compared with those of the control group(11.5±2.1)min,(3.8±2.1)ml and(3.2±1.2)d(P>0.05).The menstrual volume 3 and 6 months after surgery of the research group were(128.2±13.4)and(153.4±13.4)ml,which were all better than those of the control group(257.1±12.3)and(305.1±23.4)ml,and the difference was statistically significant(P<0.05).The incidence of postoperative complications 7.14%and recurrence rate 4.76%of the research group were lower than those of the control group 30.95%and 19.05%,and the difference was statistically significant(P<0.05).Conclusion Hysteroscopic transcervical electrocision of polyps can effectively reduce amount of hemorrhage,shorten hospitalization time,lower incidence of complications,as well as reducing the recurrence rate and improve menstrual volume.So it can be further promoted and applied in clinical practice.
