电视胸腔镜下慕丝线与腔镜切割缝合器处理肺大疱的临床比较研究

Clinical Comparison of Treatment of Pulmonary Bullae with Video-assisted Thoracoscopic Mousse Thread and Endoscopic Incision Suture Device

目的:比较电视胸腔镜下应用慕丝线与腔镜切割缝合器处理肺大疱的临床治疗效果。方法:选2016年4月至2019年4月于我院治疗的70例自发性气胸患者,随机分为两组,每组35例。观察组在电视胸腔镜下使用腔镜切割缝合器处理肺大疱,对照组在电视胸腔镜下应用慕丝线处理肺大疱。比较两组患者术中出血量、术后胸管放置时间、术后住院时间及并发症等情况。结果:两组患者术后胸管放置时间、住院时间、并发症发生率比较,差异无统计学意义(P>0.05);观察组手术操作时间短于对照组、术中出血量少于对照组、手术费用高于对照组,差异均有统计学意义(P<0.05)。结论:对于自发性气胸患者在电视胸腔镜下无论应用腔镜切割缝合器还是慕丝线治疗均有较好的效果,可根据患者的具体病情及经济条件选择适合的手术方式,实现个体化治疗。

Objective:To compare the clinical therapeutic effect of video-assisted thoracoscopic treatment of pulmonary bullae with mousse thread and endoscopic cutting suture.Methods:70 patients with spontaneous pneumothorax treated in our hospital from April 2016 to April 2019 were randomly divided into 2 groups,with 35 patients in each group.In the observation group,pulmonary bullae were treated with endoscopic cutting suture under video-assisted thoracoscopy,while in the control group,pulmonary bullae were treated with mousse thread under video-assisted thoracoscopy.Intraoperative blood loss,postoperative thoracic tube placement time,postoperative hospital stay and complications were compared between the two groups.Results:There was no significant difference in the postoperative placement time,length of stay and incidence of complications between the two groups(P>0.05).The operation time of the observation group was shorter than that of the control group,the amount of intraoperative blood loss was less than that of the control group,and the operation cost was higher than that of the control group,with statistically significant differences(P<0.05).Conclusion:For spontaneous pneumothorax patients with video-assisted thoracoscopy,both endoscopic cutting and suture and mousse thread treatment have good results.Suitable surgical methods can be selected according to patients’specific conditions and economic conditions,and individualized treatment can be realized.
