血清铁蛋白联合糖化血红蛋白检测在早期诊断妊娠糖尿病患者中的应用价值分析

Application value of serum ferritin combined with glycosylated hemoglobin in early diagnosis of gestational diabetes mellitus

目的探讨分析血清铁蛋白联合糖化血红蛋白检测在早期诊断妊娠糖尿病患者中的应用价值。方法选择2019年1月至2019年12月抚州市妇幼保健所收治的50例24~28周的妊娠糖尿病孕妇作为妊娠期糖尿病组,另将同期在本所接受产前检查的50名血糖正常孕妇作为对照组。检测对比两组孕妇的血清铁蛋白、血红蛋白、血清锌、血清铁水平及血糖代谢指标(糖化血红蛋白水平、空腹血糖水平和餐后1 h血糖水平、餐后2 h血糖水平)。结果妊娠期糖尿病组孕妇的血清铁蛋白水平、血清铁水平均显著上升(P<0.05),血清锌水平则显著降低(P<0.05),而两组血红蛋白水平比较差异无统计学意义。妊娠期糖尿病组孕妇的糖化血红蛋白水平、空腹血糖水平、餐后1 h血糖水平、餐后2 h血糖水平均显著上升(P<0.05)。结论血清铁蛋白联合糖化血红蛋白检测能够为妊娠期糖尿病的早期诊断提供科学依据和指导,具有临床推广应用价值。

Objective To explore the application value of serum ferritin combined with glycosylated hemoglobin in early diagnosis of gestational diabetes mellitus.Methods From January 2019 to December 2019,50 pregnant women with gestational diabetes mellitus from 24 to 28 weeks in Fuzhou Maternal and Child Health Center were selected as the gestational diabetes group,and 50 pregnant women with normal blood sugar received prenatal examination in the same period were selected as the control group.Serum ferritin,hemoglobin,serum zinc and serum iron levels and blood glucose metabolic indicators(glycated hemoglobin level,fasting blood glucose level and 1 h postprandial blood glucose level,2 h postprandial blood glucose level)were measured and compared between the two groups.Results The levels of serum ferritin and iron in pregnant women with gestational diabetes mellitus increased significantly(P<0.05),while the levels of serum zinc decreased significantly(P<0.05).There was no significant difference in the level of hemoglobin between the control group and gestational diabetes mellitus group.In gestational diabetes mellitus group,the levels of glycosylated hemoglobin(HbA1c),fasting blood glucose(FBG),1 h postprandial blood glucose and 2 h postprandial blood glucose increased significantly(P<0.05).Conclusion Serum ferritin combined with glycosylated hemoglobin can provide scientific basis and guidance for the early diagnosis of gestational diabetes mellitus,and has clinical application value.
