摘要
后外侧复合体(Posterolateral Complex, PLC)又称为后外侧角(Posterolateral Corner, PLC)或后外侧结构(Posterolateral Structure,PLS),损伤后可导致严重的膝关节功能障碍。孤立的PLC损伤较少见,通常伴随交叉韧带损伤发生,如果处理不当可能造成慢性关节不稳、软骨退变,甚至使交叉韧带的重建失败。近年,随着运动损伤等事件增多,PLC损伤呈上升趋势。低级别的PLC损伤常采取非手术治疗,高级别或者伴有其他韧带、肌腱的PLC损伤则需要手术干预。本文对PLC解剖学、生物力学、诊断和治疗等方面进行了综述,以期为临床PLC损伤的诊治提供参考。
The Posterolateral Complex of the knee,also known as the Posterolateral Corner or Posterolateral Structure,can lead to serious dysfunction after injury.The isolated PLC injuries are rare,frequently encountered combined with cruciate ligament ruptures,if not treated properly may cause chronic joint instability,cartilage degeneration,and even the cruciate ligament reconstruction failure.The incidence of PLC injuries has risen owing to the increase in traffic accidents and athletic traumas.Low-Grade PLC injuries can be treated with non-operative management,high-grade injuries accompanied by other structural injuries,surgical management is recommended.The purpose of this article is to review the anatomy,biomechanics,diagnosis,and treatment of PLC injuries,in order to provide more valuable reference for diagnosis and treatment in future.
