微信平台远程管理模式对社区老年高血压病患者自我管理行为的干预效果研究

Intervention Effect of Remote Management Mode of WeChat Platform on Self-management Behavior of Elderly Patients with Hypertension in Community

目的探讨微信平台远程管理模式对社区老年高血压患者自我管理行为的干预效果。方法纳入北京市丰台区社区医院于2017年1月-2018年12月间收治的94例老年高血压患者,采用电脑随机法将其划分为观察组47例与对照组47例,观察组在常规社区护理基础上结合微信平台远程管理模式,对照组采用常规社区医院护理,评价两组患者护理前后自我管理行为评分、满意度、管理模式认知程度评价。结果两组护理后自我管理行为评分均高于护理前,其中观察组护理后自我管理行为评分均高于对照组,差异有统计学意义(P<0.05)。观察组患者血压达标率为85.11%,高于对照组61.70%,差异有统计学意义(P<0.05)。管理前两组患者对微信平台远程管理模式认知程度结果显示不知率均较高(P>0.05);管理后观察组熟知率高于对照组,差异有统计学意义(P<0.05)。结论社区老年高血压患者采取微信平台远程管理模式干预可有效提高患者自我管理行为能力、血压控制率与管理模式认知度。

Objective To explore the intervention effect of remote management mode of WeChat platform on self-management behavior of elderly hypertensive patients in the community.Methods A total of 94 elderly patients with hypertension admitted to Fengtai District Community Hospital in Beijing from January 2017 to December 2018 were included.They were divided into observation group 47 and control group 47 by computer random method.On the basis of conventional community nursing combined with the remote management mode of WeChat platform,the control group adopted conventional community hospital nursing to evaluate the self-management behavior score,satisfaction degree,and management mode awareness of the two groups of patients before and after nursing.Results The scores of self-management behaviors after nursing were higher in both groups than before care,and the scores of self-management behaviors in observation group were higher than those in control group(P<0.05).The compliance rate of blood pressure in the observation group was 85.11%,which was higher than the control group by 61.70%(P<0.05).Before the two groups of patients,the degree of cognition on the remote management mode of WeChat platform showed a high rate of ignorance(P>0.05);the familiarity rate of the observation group was higher than that of the control group after management(P<0.05).Conclusion The intervention of WeChat platform remote management mode in elderly patients with hypertension in the community can effectively improve the self-management behavior ability,blood pressure control rate and management mode awareness.
