摘要
目的:分析心电图QRS波时限与慢性心力衰竭患者预后的相关性。方法:选择99例慢性心力衰竭患者作为研究对象,分为QRS波时限正常组(n=58)与QRS波时限延长组(n=41)。比较两组左室射血分数(LVEF)和血浆N末端B型利钠肽原(NT-proBNP)水平,以及随访期内的不良心血管事件发生率。结果:QRS波时限正常组LVEF为(58.83±7.42)%,NT-proBNP为(573.16±103.28)pg/mL,QRS波时限延长组分布为(41.93±6.31)%,(1302.72±316.33)pg/mL,比较有统计学意义(P<0.05)。QRS波时限正常组发生左心衰竭急性加重3例(5.2%),致命性心律失常4例(6.9%),心源性死亡0例(0),不良心血管事件发生率为12.1%;QRS波时限延长组分布为7例(17.1%),12例(29.3%),2例(4.9%),不良心血管事件发生率为51.2%,比较有统计学意义(P<0.05)。结论:心电图QRS波时限与慢性心力衰竭患者的预后密切相关,心电图QRS波时限延长提示预后不良。
Objective:To analyze the correlation between the QRS wave duration of electrocardiogram and the prognosis of patients with chronic heart failure.Methods:99 patients with chronic heart failure were selected as the research object and divided into a group with normal QRS wave duration(n=58)and a group with extended QRS wave duration(n=41).The left ventricular ejection fraction(LVEF)and plasma N-terminal pro-B-type natriuretic peptide(NT-proBNP)levels were compared between the two groups,as well as the incidence of adverse cardiovascular events during the follow-up period.Results:The LVEF of the group with normal QRS wave time was(58.83±7.42)%,NT-proBNP was(573.16±103.28)pg/mL,and the distribution of the group with extended QRS wave time was(41.93±6.31)%,(1302.72±316.33)pg/mL,which is statistically significant(P<0.05).There were 3 cases(5.2%)of acute exacerbation of left heart failure,4 cases(6.9%)of fatal arrhythmia,and 0 cases(0)of cardiac death in the normal group with QRS wave time limit.There were 7 cases(17.1%),12 cases(29.3%),and 2 cases(4.9%)in the extended wave duration group.The incidence of adverse cardiovascular events was 51.2%,which was statistically significant(P<0.05).Conclusion:The QRS wave duration of electrocardiogram is closely related to the prognosis of patients with chronic heart failure,the prolonged QRS wave duration of electrocardiogram indicates poor prognosis.
