全自动血细胞分析仪白细胞分类与人工镜检检查结果的对比分析 被引量:4

摘要 目的:比较全自动血细胞分析仪白细胞分类与人工镜检检查结果的价值。方法:选择87例行血常规检查的患者,分别行全自动血细胞分析仪与人工镜检进行白细胞分类。结果:两种方法检测嗜碱性粒细胞、淋巴细胞比较有明显差异(P<0.05),而嗜酸性粒细胞、单核细胞、中性粒细胞比较无明显差异(P>0.05)。两种方法对嗜碱性粒细胞、嗜酸性粒细胞、单核细胞、淋巴细胞、中性粒细胞的相关系数(r)分别为0.202、0.658、0.531、0.317、0.819,两种方法的嗜酸性粒细胞、单核细胞、中性粒细胞结果具有较高的一致性。以人工镜检结果作为“金标准”,全自动血细胞分析仪的灵敏度为89.50%,特异度为82.5%,假阳性率为36.2%,假阴性率为21.7%。结论:用全自动血细胞分析仪进行白细胞分类具有一定的临床应用价值,但与人工镜检结果仍有一定的差异,需要对触发复检规则的标本进行人工镜检。 Objective:To compare the value of automatic hematology analyzer white blood cell classification and artificial microscopic examination results.Methods:Eighty-seven patients undergoing routine blood tests were selected to perform automatic blood cell analyzer and artificial microscopy to classify white blood cells.Results:There were significant differences between the two methods in detecting basophils and lymphocytes(P<0.05),but there was no significant difference in eosinophils,monocytes,and neutrophils(P>0.05).The correlation coefficients(r)of the two methods for basophils,eosinophils,monocytes,lymphocytes,and neutrophils are 0.202,0.658,0.531,0.317,and 0.819,respectively.Granulocytes,monocytes,and neutrophils have high consistency.Taking the result of artificial microscopy as the"gold standard",the sensitivity of the automatic hematology analyzer is 89.50%,the specificity is 82.5%,the false positive rate is 36.2%,and the false negative rate is 21.7%.Conclusion:The classification of white blood cells with automatic hematology analyzer has certain clinical application value,but there are still some differences from the results of artificial microscopy,it is necessary to perform artificial microscopy on the specimens that trigger the re-examination rules.
