老年性下睑内翻的两种术式疗效比较

Comparison of efficacy between two kinds of operations for senile lower eyelid entropion

目的:比较老年性下睑内翻的两种术式的疗效。方法:回顾性分析2015年12月至2018年2月在神马医疗集团总医院眼科收治的老年性下睑内翻75例(98眼)的临床资料。所有患者分为两组,试验组40例(45眼),采用眼轮匝肌折叠术,对照组35例(53眼),采用缝线固定术,随访观察1年。结果:术后1个月及1年,试验组的治愈率分别为100.0%和95.6%,对照组的治愈率分别为98.1%和81.1%,术后1年试验组的治愈率高于对照组(χ^2=4.712,P=0.030)。试验组的手术时间(42.04±9.30)分钟,长于对照组的(20.31±4.77)分钟(t=14.307,P=0.000)。术后1个月及1年,试验组的满意度均高于对照组(Z=-2.766,-4.811;P=0.010,0.000)。术后1年,试验组的并发症发生率低于对照组(χ^2=5.627,P=0.018)。结论:采用眼轮匝肌折叠术治疗老年性下睑内翻,耗时较长,但患者的长期治愈率及满意度均较高,长期并发症较低。

Objective To compare the efficacy of two surgical methods for the treatment of senile lower eyelid entropion.Methods The clinical data of 98 eyes of 75 cases with senile lower eyelid entropin in Pingmei Shenma Medical Group General Hospital from Dec.2015 to Feb.2018 were analyzed retrospectively.All cases were divided into two groups.The trial group,45 eyes of 40 cases,received orbicularis muscle placation.The control group,53 eyes of 35 cases,received suture fixation.All patients were followed up and observed for 1 year.Results At 1 month and 1 year after operation,the cure rate of the trial group were 100.0%and 95.6%,respectively.And that of the control group were 98.1%and 81.1%,respectively.The cure rate of the trial group was higher than that of the control group at 1 year after operation(χ^2=4.712,P=0.030).The operating time of the trial group was(42.04±9.30)min,which of the control group was(20.31±4.77)min,the difference was statistically significant(t=14.307,P=0.000).At 1 month and 1 year after operation,the satisfaction rate of the trial group was higher than that of the control group(Z=-2.766,-4.811;P=0.010,0.000).At 1 year after operation,the incidence of complications in the trial group was lower than those in the control group(χ^2=5.627,P=0.018).Conclusion The operation time of orbicularis muscle placation for the treatment of senile lower eyelid entropin is longer,but the long-term cure rate and satisfaction of the patients are higher,the long-term complications are fewer.
