急诊应用胺碘酮治疗快速型心律失常疗效研究

Study on the efficacy of amiodarone in the treatment of tachyarrhythmia

目的探讨急诊应用胺碘酮治疗快速型心律失常的疗效。方法收集2015年1月~2016年5月我院收治的69例快速型心律失常患者,按随机数字表法随机分组,对照组(常规治疗方法)34例,观察组(对照组治疗的基础上用胺碘酮治疗)35例,比较临床疗效、治疗前后收缩压、舒张压及心率变化,不良反应发生率。结果观察组显效19例、有效14例,总有效率为94.29%,高于对照组,差异有统计学意义(P<0.05)。两组治疗后收缩压、舒张压、心率低于治疗前,差异有统计学意义(P<0.05);治疗后观察组收缩压、舒张压、心率低于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率为5.71%,对照组为20.59%,观察组不良反应发生率低于对照组,两组差异有统计学意义(P<0.05)。结论急诊应用胺碘酮治疗快速型心律失常效果满意,可提高临床疗效,降低血压及心率,不良反应发生率低,安全性高,建议在临床推广应用。

Objective To explore the effect of amiodarone in the treatment of tachyarrhythmia.Methods Sixty-nine patients with tachyarrhythmias admitted in our hospital from January 2015 to May 2016 were selected.They were randomly divided into the control group(receiving a regular treatment,n=34)and the observation group(amiodarone was used in addition to the control group,n=35)using the random number table.Clinical efficacy,pre-and post-treatment changes in systolic blood pressure(SBP),diastolic blood pressure(DBP)and heart rate(HR),and incidence rates of adverse reaction were compared between the two groups.Results In the observation group,19 patients achieved excellent therapeutic efficacy and 14 satisfactory efficacy with the total effective rate of 94.29%which was higher than that in the control group,with statistically significant differences(P<0.05).In both two groups,post-treatment SBP,DBP and HR were lower than the pre-treatment values with statistically significant differences(P<0.05).Post-treatment SBP,DBP and HR in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).The incidence rate of adverse reaction in the observation group was 5.71%,much lower than the rate in the control group(20.59%),with statistically significant differences(P<0.05).Conclusion The effect of amiodarone in the treatment of tachyarrhythmias is satisfactory.It can improve clinical efficacy and reduce blood pressure and HR with low incidence rates of adverse reaction and high safety,which is recommended for wide application in clinic.
