再造耳廓形态不良治疗策略及手术方法的研究进展

Research progress of surgical treatment for poorly formed reconstructed auricle

耳再造术经过长期的探索和应用,已发展为非常成熟的术式,但仍存在许多术后并发症,若处理不及时就可能导致手术失败,造成再造耳廓形态不良。作者对再造耳廓形态不良的判断方法、原因作了归纳总结,对其手术治疗方法和策略进行了详细地综述,如有些情况通过局部小的修复手术即可得到改善,而情况较严重时原有的耳支架必须完全取出,并依据患者的术区具体情况应用耳后皮瓣、筋膜瓣、颞浅血管岛状筋膜瓣等和支架重新雕刻植入技术进行再次修复。

After a long period of exploration and application,reconstruction of auricle has become a maturesurgical procedure.There are still some postoperative complications,which may lead to the failure of surgery if not treated in time,resulting in unsatisfactoryappearance of reconstructed auricle.The authors summarized the methods of evaluation and causes of failed auricle reconstruction,and reviewed the surgical treatment methods and strategies in details.Mild cases can be improved by local repair surgery,but if the reconstructed auricle is seriously deformed,the original ear framework must be completely removed.According to the specific condition of the patient’s operative area,the posterior auricular flap,fascia flap,superficial temporal vascular island fascia flap and ear framework re-carving and implantation technique were used for salvage surgery.
