超高龄患者进行白内障超声乳化吸除联合人工晶状体Ⅰ期植入术的临床研究

Clinical study of phacoemulsification combined with intraocular lens implantation in ultra elderly patients with catarcats

目的 观察超高龄白内障患者超声乳化联合人工晶状体植入术后视力及并发症发生情况,总结临床应用价值.方法 回顾性分析100例超高龄白内障患者的临床资料,按是否合并糖尿病分为观察组(55例)和对照组(45例),均行白内障超声乳化吸除联合人工晶状体Ⅰ期植入术,观察对比两组患者手术前后眼压和裸眼视力及术后并发症发生率.结果 两组患者术后1 d眼压均较术前升高(P<0.05),术后1、3、8周的眼压与术前接近(P>0.05);观察组术后1 d、术后1、3、8周同期眼压均高于对照组(P<0.05).两组患者术后1 d、术后1、3、8周的裸眼视力均较术前升高(P<0.05);观察组术后3、8周同期裸眼视力均低于对照组(P<0.05).观察组术后并发症发生率高于对照组(P<0.05).结论 超高龄白内障患者行超声乳化联合人工晶状体植入术,利于提高患者术后视力和降低眼压,值得临床推广应用,但合并糖尿病患者的术后并发症较多.

Objective To observe the visual acuity and complications after phacoemulsification combined with intraocular lens implantation in ultra elderly patients with catarcats,and summarize the clinical application value.Methods Retrospective analyzed the clinical data of one hundred cases of ultra elderly cataract patients,who were divided into the the observation group(n=55)and control group(n=45)according to whether they had diabetes.All the patients underwent phacoemulsification combined with intraocular lens implantation at phase I.The intraocular pressure,visual acuity of naked eyes and postoperative complications of the two groups before and after operation were compared.Results Intraocular pressure was increased in both groups at 1 day after surgery compared with that before surgery(P<0.05),and was close to that before surgery at 1,3 and 8 weeks after surgery(P>0.05).Intraocular pressure in the observation group was higher than that in the control group at 1 day,1,3 and 8 weeks postoperatively(P<0.05).The visual acuity of the naked eye was increased in both groups at 1 day,1,3 and 8 weeks after surgery(P<0.05).The visual acuity of the observation group was lower than that of the control group in 3 and 8 weeks after operation(P<0.05).The incidence of pos toperative complications in the observ ation group was higher than that in the control group(P<0.05).Conclusion Phacoemulsification combined with intraoc-ular lens implantation is beneficial to improve postoperative visual acuity and reduce intraocular pressure in cataract patients with ad-vanced age,which is worthy of clinical application,but patients with diabetes have more postoperative complications.
