减重代谢手术效果综合评价系统(BAROS)的引进和初步应用

目的探讨减重代谢手术效果综合评价系统(Bariatric Analysis and Reporting Outcome System,BAROS)在单纯性肥胖患者中的初步应用价值。方法对2019年1~6月我科42例单纯性肥胖接受胃袖状切除术患者,采用中文版BAROS观察术后6个月额外体重降低百分比(percentage of extra weight loss,%EWL)、合并症、生活质量及并发症和再手术情况,按照中文版BAROS总分评级标准评价手术的效果。结果42例术后6个月%EWL为52.38±27.74(19.06~123.03)。术后6个月体重、BMI、腹围、体脂比、空腹血糖、甘油三酯等指标与术前相比均明显降低(P<0.05),合并症在术后都得到改善,其中高脂血症缓解率为90.0%(18/20),睡眠呼吸暂停缓解率为60.7%(17/28)。生活质量评定为“好”和“非常好”的患者占66.7%(28/42)。4例(9.5%)出现次要并发症。BAROS总分评级标准“良好”以上的患者占88.1%(37/42),无“失败”的患者。结论中文版BAROS全面、量化、简单易用,可用于肥胖患者减重代谢手术的效果评价,便于与国际同行交流。

Objective To explore the value of the Bariatric Analysis and Reporting Outcome System(BAROS)for simple obesity patients.Methods Forty-two obese patients after laparoscopic sleeve gastrectomy(LSG)were enrolled.The Chinese version BAROS was used.The percentage of extra weight loss(%EWL),co-morbidities,quality of life,complications and re-operation conditions were calculated.The effects of operation were evaluated by using the Chinese version BAROS.Results Of the 42 patients,the%EWL was 52.38±27.74(range,19.06-123.03).The weight,BMI,waist circumference,body fat percentage,fasting blood glucose and triglyceride of post-operational patients were significantly lower than that of pre-operational levels(P<0.05).The co-morbidities were improved after operation.The remission rates of hyperlipidemia and sleep apnea were 90.0%(18/20)and 60.7%(17/28),respectively.Twenty eight patients’quality of life were rated as GOOD and VERY GOOD.There were 4 patients(9.5%)with minor complications.The results of BAROS showed that 88.1%(37/42)of the patients were rated as GOOD and above,and no patients was rated as FAILURE.Conclusion The Chinese version BAROS is comprehensive,quantitative and operable,and can be used to analyze the effect of bariatric surgery in obese patients and to communicate with international colleagues.
