诱发性呼吸训练器在胸部外伤合并慢性阻塞性肺疾病患者中的应用

Effect of training with incentive spirometry on prognosis of thoracic trauma patients with chronic obstructive pulmonary disease

目的在胸部外伤合并慢性阻塞性肺疾病(obstructive pulmonary disease,COPD)患者治疗中探讨使用诱发性呼吸训练器(incentive spirometry,IS)的作用。方法选择2017年4月—2018年3月在本院急诊科住院的胸部外伤合并COPD患者共86例,根据入院时间编号随机分为对照组44例(奇数)和观察组(偶数)42例,对照组采用常规护理方法,观察组在常规护理基础上联合使用IS进行呼吸功能锻炼。比较两组患者的肺炎发生率,呼吸机使用率、住院时间和死亡率。结果相比对照组,观察组患者肺炎发生率下降,呼吸机使用率降低,住院时间缩短,两组比较,差异具有统计学意义(均P<0.05),但两组死亡率差异无统计学意义(P>0.05)。结论IS呼吸训练有助于改善胸部外伤合并COPD患者的肺功能,减少肺炎发生,值得临床推广应用。

Objective To investigate the effect of training with incentive spirometry(IS)on the prognosis of trauma patients with chronic obstructive pulmonary disease(COPD).Methods A total of 86 thoracic trauma patients with COPD were included in the emergency department of the Second Hospital of Zhejiang University School of Medicine.According hospitalized number,the patients were randomly divided into control group with even number(n=44)and observation group(n=42)with odd number.The former was treated with conventional nursing strategies to improve respiratory function,and the latter with IS-based training along with conventional nursing.The two groups were compared in terms of incidence of pneumonia,use of ventilator,length of hospital stay and mortality.Results Compared with the control group,the incidence of pneumonia and use of ventilator were significantly lower,and the hospital stay was significantly shorter in the IS group(P<0.05).There was no statistically significant difference in the mortality of patients between them(P>0.05).Conclusion IS-based respiratory training is helpful for the improvement of pulmonary function,reduction of incidence of pneumonia,and shortening of hospital stay.
