腹腔镜术后坤泰胶囊联合促性腺激素释放激素激动剂治疗卵巢子宫内膜异位囊肿的临床效果

Clinical efficacy of Kuntai Capsules combined with gonadotropin-releasing hormone agonist for treatment of ovarian endometriosis cyst after laparoscopic surgery

目的探讨腹腔镜术后坤泰胶囊联合促性腺激素释放激素激动剂(GnRH-a)治疗卵巢子宫内膜异位囊肿的疗效。方法将行腹腔镜手术治疗的125例卵巢子宫内膜异位囊肿患者分为观察组(n=63)及对照组(n=62)。术后观察组给予坤泰胶囊联合GnRH-a治疗,对照组仅予以GnRH-a治疗。比较两组总有效率、治疗前后性激素[包括血清促卵泡激素(FSH)、雌激素、抗苗勒氏管激素(AMH))]及糖类抗原125(CA125)水平,并记录两组患者不良反应、复发发生情况及妊娠情况。结果观察组总有效率高于对照组(P<0.05),而失眠、潮热发生率低于对照组(P<0.05)。治疗后两组AMH、雌激素及CA125水平较治疗前降低,而FSH较治疗前升高(P<0.05),但观察组雌激素、AMH水平高于对照组,且FSH及CA125水平低于对照组(均P<0.05)。两组患者复发率及妊娠率比较,差异均无统计学意义(均P>0.05)。结论对于腹腔镜术后的卵巢子宫内膜异位囊肿患者,与单纯使用GnRH-a比较,联合坤泰胶囊可有效地减轻GnRH-a引起的低雌激素症状,并改善卵巢的储备功能。

Objective To investigate the efficacy of Kuntai Capsules combined with gonadotropin-releasing hormone agonist(GnRH-a)for the treatment of ovarian endometriosis cyst after laparoscopic surgery.Methods A total of 125 patients with ovarian endometriosis cyst treated by laparoscopic surgery were divided into observation group(n=63)and control group(n=62).The observation group was treated with Kuntai Capsules combined with GnRH-a after operation,whereas the control group was treated with GnRH-a alone after operation.Total effective rate,pre-and post-treatment sex hormones(including serum follicle-stimulating hormone[FSH],estrogen,anti-Müllerian hormone[AMH])and carbohydrate antigen 125(CA125)levels were compared between the two groups,and adverse reactions,incidence of recurrence,and pregnancy outcome were recorded in both groups.Results The total effective rate was higher and the incidence rates of insomnia and hot flashes were lower in the observation group than in the control group(all P<0.05).After treatment,the levels of AMH,estrogen and CA125 were decreased and FSH level was increased in both groups in comparison with the pre-treatment levels(P<0.05),furthermore,the observation group exhibited higher estrogen and AMH levels and lower FSH and CA125 levels than the control group(all P<0.05).There was no statistically significant difference in recurrence rate or pregnancy rate between the two groups(all P>0.05).Conclusion For patients with ovarian endometriosis cysts following laparoscopic surgery,compared with GnRH-a therapy alone,combined therapy of Kuntai Capsules and GnRH-a can effectively ameliorate low estrogen symptoms caused by GnRH-a and improve ovarian reserve function.
