摘要
目的探讨早期食管癌患者外周血中微小RNA(miRNA)-15b和miRNA-34c表达水平及其临床意义。方法纳入113例早期食管癌患者(食管癌组)及113例健康者(对照组)。检测两组血清miRNA-15b、miRNA-34c表达水平。分析miRAN-15b和miRAN-34c与早期食管癌发生及临床病理特征关系,并评估两者对早期食管癌诊断价值。结果食管癌组血清miRNA-15b、miRNA-34c表达水平均低于对照组(均P<0.05)。低分化早期食管癌患者血清miRNA-15b、miRNA-34c水平低于高、中分化患者(均P<0.05),TNM分期Ⅱ期患者血清miRNA-15b、miRNA-34c水平低于TNM分期Ⅰ期患者(均P<0.05)。miRNA-15b相对表达水平≤0.78、miRNA-34c相对表达水平≤0.69是早期食管癌发生的危险因素(均P<0.05)。外周血血清miRNA-15b相对表达水平与miRNA-34c相对表达水平联合预测早期食管癌的曲线下面积为0.949(P<0.001),敏感度为93.00%,特异度为81.80%,均高于单一指标。结论早期食管癌患者血清中miRNA-15b与miRNA-34c均低表达,这可能与食管癌发生与发展有关。两者联合诊断早期食管癌价值较高。
Objective To investigate the expression levels and clinical significance of peripheral blood microRNA(miRNA)-15b and miRNA-34c in patients with early esophageal cancer.Methods A total of 113 patients with early esophageal cancer(esophageal cancer group)and 113 healthy individuals(control group)were enrolled.The expression levels of serum miRNA-15b and miRNA-34c were detected in the two groups.The relationship of miRNA-15b and miRNA-34c with incidence and clinicopathologic characteristics of early esophageal cancer was analyzed,and their diagnostic values for early esophageal cancer were analyzed.Results The expression levels of serum miRNA-15b and miRNA-34c in the esophageal cancer group were lower than those in the control group(all P<0.05).The levels of serum miRNA-15b and miRNA-34c in patients with lowly differentiated early esophageal cancer were lower than those in patients with highly or moderately differentiated early esophageal cancer(all P<0.05),and the levels of serum miRNA-15b and miRNA-34c in patients with TNM stageⅡwere lower than those in patients with TNM stageⅠ(all P<0.05).The relative expression level of miRNA-15b≤0.78 and the relative expression level of miRNA-34c≤0.69 were the risk factors for developing early esophageal cancer(all P<0.05).For early esophageal cancer prediction,the area under the curve of combined detection of the relative expression levels of serum miRNA-15b and miRNA-34c in peripheral blood was 0.949(P<0.001),with a sensitivity of 93.00%and a specificity of 81.80%,higher than those of the single index.Conclusion miRNA-15b and miRNA-34c are lowly expressed in serum of patients with early esophageal cancer,which may correlate with the development and progression of early esophageal cancer.The combination of these two indices has higher diagnostic value for early esophageal cancer.
