摘要
目的 探讨改良大骨瓣减压术中渐进性减压对重症颅脑损伤患者颅内压及远期预后的影响.方法 选取2016年10月至2018年10月收治的重症颅脑损伤患者78例为研究对象,以随机法将其分为对照组和观察组,各39例.所有患者均接受改良大骨瓣减压术,对照组实施常规减压,观察组实施渐进性减压,比较两组的治疗效果.结果 术后1、5 d,观察组的GCS评分明显高于对照组(P<0.05).术后1、5 d,观察组的颅内压明显低于对照组(P<0.05).观察组的远期预后恢复良好率高于对照组,死亡率低于对照组(P<0.05).结论 改良大骨瓣减压术中渐进性减压治疗重症颅脑损伤能够促进患者神经功能恢复,改善远期预后,值得推广.
Objective To investigate the effects of progressive decompression during modified large bone flap decompression on intracranial pressure and long-term prognosis in patients with severe craniocerebral injury.Methods A total of 78 patients with severe craniocerebral injury admitted from October 2016 to October 2018 were selected as the study objects.The patients were divided into control group and observation group by random method,with 39 cases in each group.All patients were treated with modified large bone flap decompression,the control group was given conventional decompression,the observation group was given progressive decompression,the treatment effects of the two groups were compared.Results On the 1st and 5th day after operation,the GCS score of the observation group were significantly higher than those of the control group(P<0.05).On the 1st and 5th day after operation,the intracranial pressure in the observation group were significantly lower than those in the control group(P<0.05).The recovery good rate of long-term prognosis of the observation group was higher than that of the control group,and the mortality was lower than that of the control group(P<0.05).Conclusion Progressive decompression during modified large bone flap decompression in the treatment of patients with severe craniocerebral injury can promote the recovery of neurological function and improve the long-term prognosis,which is worth popularizing.
