早期八风八邪穿刺排毒法对蝮蛇咬伤肢体肿胀及相关炎性因子水平的影响

Effects of Early Puncture and Detoxification of Eight Winds and Eight Pathogenic Factors on Limb Swelling and Related Inflammatory Factors in Agkistrodon Bite

目的:探讨早期八风八邪穿刺排毒法对蝮蛇咬伤肢体肿胀及相关炎性因子的影响。方法:100例蝮蛇咬伤患者分为对照组和试验组各50例,对照组给予普通局部切开及常规治疗,试验组给予八风八邪穿刺及常规治疗,于治疗前及治疗后3、6 d记录伤肢肿胀程度,检测血清5-HT、组胺、CRP、TNF-α、IL-6水平并统计分析。结果:与本组治疗前比较,试验组治疗后3、6 d患肢肿胀明显减轻,且较对照组相应时点改善更明显;2组治疗后3 d自身比较,对照组各结果差异无统计学意义,试验组CRP、TNF-α、IL-6水平降低,对照组治疗6 d后CRP、5-HT、TNF-α、IL-6及试验组组胺水平降低,试验组CRP、5-HT、TNF-α、IL-6水平降低更明显;与对照组比较,试验组治疗后6 d 5-HT、组胺及治疗后3 d、6 d CRP、TNF-α、IL-6水平降低均更明显。结论:早期八风八邪穿刺法通过减压排毒加速患肢肿胀消退,更快缓解全身炎症反应,实现解毒目的。

Objective:To investigate the effect of early puncture and detoxification of eight winds and eight pathogenic factors on limb swelling and expression of inflammatory factors in peripheral blood of Agkistrodon bite.Methods:100 patients with Agkistrodon bites were randomly divided into the experimental group and control group,50 cases in each group,the control group was given methods of ordinary local incision and conventional treatments,on the basis of routine treatment in the control group,the experimental group was given early puncture and detoxification of eight winds and eight pathogenic factors,the degree of limb swelling was recorded before beginning treatment、3th days and 6th days after treatment,and the levels of CRP、5-HT、histamine、TNF-αand IL-6 were detected,statistical analysis and comparison were carried out.Results:Compared with before treatment,the swelling of affected limbs in the experimental group decreased significantly at 3th days and 6th days after treatment,and the improvement was more obvious than that in the control group;Self-comparison between the two groups,there was no significant difference in the results of the control group,the levels of CRP,TNF-αand IL-6 in the experimental group were significantly lower than those before treatment;on 6th days after treatment,the levels of CRP,5-HT,TNF-αand IL-6 in the control group and histamine in the experimental group were all decreased;The levels of CRP,5-HT,TNF-αand IL-6 in the experimental group were significantly decreased;compared with the control group,the levels of 5-HT,histamine on 6th days after treatment and CRP,TNF-αand IL-6 on 3th,6th days after treatment decreased more significantly in the experimental group.Conclusion:The method of early puncture and detoxification of eight winds and eight pathogenic factors can accelerate the helieving of swelling of the injured limb through decompression,and more quickly relieve the systemic inflammatory response,and then achieve detoxification purposes.
