硝普钠联合多巴胺治疗顽固性心力衰竭的临床效果观察

Clinical effect of sodium nitroprusside combined with dopamine in the treatment of intractable heart failure

目的观察硝普钠联合多巴胺治疗顽固性心力衰竭的临床效果。方法选取2017年4月-2019年4月四会市人民医院收治的顽固性心力衰竭患者112例,采用掷骰子单双法分为对照组和观察组,每组56例。对照组接受常规抗心力衰竭治疗,观察组在对照组基础上给予硝普钠联合多巴胺治疗。比较2组心功能指标、治疗效果、不良反应发生情况。结果治疗1周后,观察组6 min步行试验、左心室射血分数指标均高于对照组,血浆脑利钠肽水平低于较对照组(P<0.01);观察组总有效率为92.86%,高于对照组的66.07%(χ~2=12.311,P<0.01);观察组不良反应总发生率为8.93%,低于对照组的39.29%(χ~2=14.104,P<0.01)。结论硝普钠联合多巴胺治疗顽固性心力衰竭的疗效确切,可改善患者心功能指标,且安全性高,值得临床推广应用。

Objective Observe the clinical effect of sodium nitroprusside combined with dopamine in the treatment of intractable heart failure.Methods 112 patients with refractory heart failure admitted to People’s Hospital of Sihui City from April 2017 to April 2019 were selected and divided into a control group and an observation group,56 cases in each group.The control group received conventional anti-heart failure treatment,and the observation group was given nitroprusside combined with dopamine on the basis of the control group.The cardiac function index,therapeutic effect,and adverse reactions were compared between the two groups.Results After 1 week of treatment,the observation group’s 6-minute walk test and left ventricular ejection fraction were higher than the control group,and the plasma brain natriuretic peptide level was lower than that of the control group(P<0.01).The total effective rate in the observation group was 92.86%,which was higher than 66.07%in the control group(χ~2=12.311,P<0.01).The total incidence of adverse reactions in the observation group was 8.93%,lower than 39.29%in the control group(χ~2=14.104,P<0.01).Conclusion Sodium nitroprusside combined with dopamine has a definite effect on the treatment of refractory heart failure,it can improve the patient’s cardiac function index,and it has high safety and is worthy of clinical promotion and application.
