4R危机管理模式在骨盆骨折合并休克患者中的应用

Application of 4R Crisis Management Model in the Nursing of Patients with Pelvic Fracture Complicated with Shock

目的:探讨4R危机管理模式在骨盆骨折合并休克患者中的应用方法及效果。方法:将2018年7月~12月我院收治的46例骨盆骨折合并休克患者纳入对照组,实施常规护理管理模式;将2019年1月~6月我院收治的48例骨盆骨折合并休克患者纳入研究组,实施4R危机管理模式;比较两组护理效果。结果:两组抢救时间、抢救成功率、住院时间、下床活动时间、骨折愈合时间、并发症发生率比较差异有统计学意义(P<0.05)。两组出院时生活质量综合评定问卷(GQOLI-74)评分高于入院时(P<0.05),研究组出院时GQOLI-74评分高于对照组(P<0.05)。结论:4R危机管理模式可有效改善骨盆骨折合并休克患者的抢救情况及预后,加快康复进程,提升患者生活质量。

Objective:To explore the application and effect of 4R crisis management model in patients with pelvic fracture complicated with shock.Methods:Forty-six patients with pelvic fracture and shock admitted to our hospital from July 2018 to December were included in the control group,and routine nursing management was implemented.Forty-eight patients with pelvic fracture complicated with shock admitted to our hospital from January 2019 to June were included in the study group and 4R crisis management model was implemented.The nursing effect of the two groups was compared.Results:There were statistically significant differences in rescue time,rescue success rate,hospital stay,time of getting out of bed,fracture healing time and complication rate between the two groups(P<0.05).The GQOLI-74 scores of the two groups at discharge were higher than that at admission(P<0.05),and the GQOLI-74 score of the study group at discharge was higher than that of the control group(P<0.05).Conclusion:4R crisis management model can effectively improve the rescue situation and prognosis of patients with pelvic fracture combined with shock,accelerate the recovery process and improve the quality of life of patients.
