摘要
目的探讨血清甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL)以及高密度脂蛋白(HDL)等指标的变化在结肠癌诊疗中的临床应用价值。方法采用美国贝克曼库尔特公司BECKMAN COULTER Chemistry Analyzer AU5800仪器及配套试剂对西安市第一医院2012-2018年收治的68例结肠癌患者(结肠癌组)及50例正常健康者(对照组)的血清TG、TC、LDL、HDL进行测定,同时采用罗氏公司Cobas e601电化学发光免疫分析仪及配套试剂对结肠癌组患者的血清癌胚抗原(CEA)指标进行测定,统计分析检测结果。对31例结肠癌组患者进行60个月的随访,分析血脂与患者5年生存率的相关性。结果结肠癌组患者血清TG、TC、LDL、HDL检测结果在不同性别和年龄比较差异均无统计学意义(P>0.05);结肠癌组患者的血清TG水平为(3.12±1.90) mmol/L,与对照组的(2.88±1.31) mmol/L比较,差异无统计学意义(P>0.05);结肠癌组和对照组受检者的血清TC [(4.67±1.62) mmol/L vs (2.99±1.59) mmol/L]、LDL [(4.07±1.71) mmol/L vs (2.78±1.87) mmol/L]、HDL [(1.39±1.15) mmol/L vs (2.01±1.25) mmol/L]水平比较差异均有统计学意义(P<0.05);随着TC、LDL水平升高,结肠癌患者的5年累计生存率降低,差异均有统计学意义(P<0.05);相关性分析结果显示,TC、LDL水平与肿瘤标志物CEA呈正相关(r=0.668 1、0.579 1,P<0.05)。结论血清TC、LDL水平与结肠癌的发生发展密切相关,有望成为预测结肠癌发生的一个有效检测指标。
Objective To investigate the clinical value of serum triglyceride(TG),total cholesterol(TC),low density lipoprotein(LDL),and high density lipoprotein(HDL)in the diagnosis and treatment of colon cancer.Methods The serum TG,TC,LDL,and HDL of 68 colon cancer patients(colon cancer group)and 50 normal healthy patients(control group)admitted to Xi'an No.1 Hospital from 2012 to 2018 were measured by BECKMAN COULTER Chemistry Analyzer AU5800 and matching reagent.The serum carcino-embryonic antigen(CEA)of colon cancer group was determined by using Cobas e601 electrochemical luminescence immunoanalyzer and matching reagent,and the results were statistically analyzed.Thirty-one patients in the colon cancer group were followed up for 60 months to analyze the correlation between blood lipids and 5-year survival rate.Results There were no statistically significant differences in serum TG,TC,LDL,and HDL levels of colon cancer patients in different genders and ages(P>0.05).The serum TG level of patients in the colon cancer group was(3.12±1.90)mmol/L,which was not statistically significant compared with(2.88±1.31)mmol/L in the control group.Serum TC,LDL,and HDL levels showed statistically significant differences between the colon cancer group and the control group(P<0.05):TC,(4.67±1.62)mmol/L vs(2.99±1.59)mmol/L;LDL,(4.07±1.71)mmol/L vs(2.78±1.87)mmol/L;HDL,(1.39±1.15)mmol/L vs(2.01±1.25)mmol/L.The 5-year cumulative survival rate of colon cancer patients decreased with the increase of TC and LDL level(P<0.05).Correlation analysis results showed that the levels of TC and LDL were positively correlated with the tumor marker CEA(r=0.6681,0.5791,P<0.05).Conclusion Serum levels of TC and LDL are closely related to the occurrence and development of colon cancer,which is expected to be an effective indicator to predict the occurrence of colon cancer.
