新辅助化疗在腹腔镜手术治疗老年结直肠癌患者中的应用效果观察

Application Efficacy Observation of Neoadjuvant Chemotherapy in Laparoscopic Surgery for Senior Colorectal Cancer Patients

为观察新辅助化疗在腹腔镜手术治疗老年结直肠癌患者的应用效果,将老年结直肠癌患者100例随机分为对照组和治疗组,每组50例。对照组患者在腹腔镜手术前接受常规治疗,治疗组患者在腹腔镜手术前接受新辅助化疗,比较2组患者胃肠道功能恢复时间、肿瘤血清学标志物水平、一般指标及术后并发症发生率。结果显示,治疗组患者术后肛门排气时间、首次排便时间、首次进食固体食物时间、肠鸣音恢复正常时间均短于对照组(P<0.05),腹腔镜手术前及术后治疗组患者CA242、CA19-9、IGF-Ⅱ水平均低于对照组(P<0.05),治疗组患者腹腔镜手术时间、术中出血量、住院时间均短于或少于对照组(P<0.05),治疗组患者术后并发症发生率低于对照组(P<0.05)。结果表明,腹腔镜手术前应用新辅助化疗治疗老年结直肠癌具有促进患者术后胃肠道功能恢复,降低肿瘤血清学水平,缩短手术时间、住院时间,减少术中出血量,减少并发症等优点。

This study was to observe the application efficacy of neoadjuvant chemotherapy in laparoscopic surgery for senior colorectal cancer patients,randomly divided 100cases of senior colorectal cancer into control group(50cases,received routine treatment before laparoscopic procedure)and treatment group(50 cases,intead of neoadjuvant chemotherapy);then,compared both groups'time for gastrointestinal function recovered,the level of tumor serum marker,normal norms,and postoperative complication incidence.As results,after procedure in the time for flatus,for first-time eating solid food,and for bowel sound returned normal;in the levels of CA242,CA19-9and IGF-Ⅱbefore and after procedure,in laparoscopic operating time,in intraoperative bleeding volume,and in hospitalization time;and in postoperative complication incidence treatment group was respectively shorter,lower,shorter or less,and lower than control group(all,P<0.05).Results show that before laparoscopic procedure performing neoajuvant chemotherapy on senior colorectal cancer patients has following merits such as promoting postoperative gastrointestinal function recovered,reducing the level of tumor serum marker,shorten operating and hospitalization time,as well decreasing intraoperative bleeding volume and complication,etc.
