中性粒细胞与淋巴细胞比值监测溃疡性结肠炎的意义分析

Significance of the Ratio of Neutrophils/Lymphocytes for Monitoring Ulcerative Colitis

为分析中性粒细胞与淋巴细胞比值监测溃疡性结肠炎的意义,选取我院2017年1月至2018年12月收治的溃疡性结肠炎患者80例作为研究对象。参考蒙特利尔分类对溃疡性结肠炎部位进行分区分为E127例,E230例,E323例。根据改良Mayo评分结果进行分组,2分及以下为缓解组,3~5分为轻度活动组(简称轻度组),6~10分为中度活动组(简称中度组),11~12分为重度活动组(简称重度组),例数分别为10例、24例、25例和21例。所有患者均行中性粒细胞、淋巴细胞检查,计算比值。结果显示,4组患者N、L、NLR水平比较差异有统计学意义(P<0.05),E1、E2、E3患者的N、L、NLR水平比较差异有统计学意义(P<0.05)。多因素非条件Logistic分析发现,N、L、NLR与溃疡性结肠炎活动相关(OR=1.574、1.594、1.745,P<0.05)。NLR判定溃疡性结肠炎活动具有较好的临床价值,敏感度为55.59%、特异度65.74%、截点值2.36,曲线下面积0.874。结果表明,中性粒细胞与淋巴细胞比值能反映溃疡性结肠炎病情及病位,有利于制定个性化治疗方案。

This study was to explore the significance of Neutrophils/Lymphocytes(N/L)ratio(NLR)for monitoring ulcerative colitis(UC),enrolled 80UC patients treated in authors'hospital(2017-01-2018-12)as research objects,and took sector of them:E127cases,E230cases,and E323cases,according to Montelier classification;then,based on modified Mayo score results grouped them:relif group(2≤points)of 10 cases,slight active group(3-5points)of 24cases,moderate active group(6-10points)of 25cases,and severe active group(11-12points)of 21cases;all the patients received the detection with N and L,the N/L ratio(NLR)was estimated.As results,in N,L and NLR among above four groups there was statistical difference(P<0.05),and the patients of E1,E2and E3had statistical difference(P<0.05);multi-variate logistic analysis showed N,L and NLR were related to activity of UC(OR=1.574,1.577 and 1.745,P<0.05);in judging UC activity NLR had better clinical value,its sensitivity was 55.59%,specificity was 65.74%,cut-point value was 2.36,area under the curve(AUC)was 0.874.Results show that N/L ratio(NRL)can reflect UC condition and sites out,which is beneficial to develop personalized modality.
